Scope of Position/Position Summary
The IRP Patient Care Resource Manager, PCRM, is a nurse case manager thatfacilitatespatients through the health care system to include pre-admission throughpost dischargecoordination of care, resources and servicesutilizingthe nursing process. The PCRM monitors and coordinates the patient plan of care to enhance the timeliness of patient care decisions and throughput and to enhance care quality and efficiency and ensure efficient resource utilization. The PCRMidentifiesand removes barriers to progression of the plan of care for each patient. The PCRM collaborates with physicians,nursesand other health care workers to manage thepatienttransition to the next level of care. The PCRM is thecentral informationsource for third party payers, communicating the plan of care, medical necessity, and gaining approval forpost dischargecare.The PCRM, through the use of evidence-based criteria, assists the physicians in ensuring the appropriate level of care for each patient.
Minimum Education Required
Bachelor's Level Degree or equivalent combination of education and experience.
Required Qualifications
Bachelor’s degree, Masters preferred. RN licensurerequired;fiveyearsrecent clinical experience applicable to the population preferred; knowledge of clinical practice guidelines, computer data bases, and payer protocols desired.
