Aledade Care Solutions (ACS) is the patient-facing arm of Aledade. ACS works in collaboration with Aledade partner practices to improve patient outcomes, while reducing health care costs. The ACS innovation team is launching a new program focused on improving transitions of care for Aledade patients who are in post-acute facilities. We are actively recruiting a SNF Engagement Manager (Transitional Care Lead) to become a foundational team member of this program. This role will initially work directly with skilled nursing facilities to coordinate the discharge plan for Aledade ACO patients and connection back to their PCP in the community.
The ideal candidate for this role is a self-starter with a track record of working with skilled nursing facilities, other post-acute care providers, or value based care programs. This role allows the candidate to join ACS’ fast growing Innovation and Partnerships team, which focuses on testing and scaling new value based interventions with the potential to change the U.S. healthcare system.
We are flexible with respect to geographic location of employees within the U.S.A. The ideal candidate will be comfortable working remotely and will be able to travel from time to time to establish or maintain relationships with high-volume facilities.
Duties and Responsibilities:
- Develop and maintain overall Aledade business relationships with key leaders in Skilled Nursing facilities prioritized for this program
- Lead and/or support data review meetings with key Skilled Nursing Facility staff
- Track and monitor SNF patients, including care plan, length of stay, and communication between facilities and providers via 3rd party platform and other tools available
- Work on a cross-functional team to establish and improve our SNF transitions of care program
- Use an evidence-based, data-driven approach to optimize Value-Based outcomes for SNF patients
Required Education and Experience:
- Bachelor’s degree
- Deep expertise in skilled nursing facility transitions (ie admissions, administration)
- Clinical background (RN, LMSW, LCSW, PT/OT, or similar)
Preferred Education, Skills, and Experience:
- Minimum 3 years experience working in a value based care environment
- Basic knowledge of performance improvement concepts and EMR documentation
- Experience working with post-acute care providers (i.e. home care agencies, hospitals, and PCP’s) and other post-acute resources
- Experience working with cross-functional teams to implement or refine programs
