Starting Point Nursing Services

The Transitional Care Tele Triage LPN provides services to the assigned population in accordance with protocols. The transitional primary care LPN serves as a resource for telehealth patients and providers. You will serve as a coordinator for this virtual visit program.

• Schedule needed virtual visits in coordination with telehealth provider
• Receive and upload records into HCHB, starting client chart
• Send RingCentral link to designated HH clinician and client for scheduled visit
• Ensure compliance with state/local requirements through proper documentation
• Participate in performance improvement activities and maintain ongoing clinical knowledge
• Document in real time
• Actively participate in applicable meetings as requested
• May complete SNF Triage LPN duties as needed

• 2 years of experience in a clinical healthcare setting
• High school diploma/ GED required; Associate’s Degree or some college preferred
• Licensed Practical Nurse (LPN) with an active license; compact license preferred.
• Experience with electronic medical record software and proficient use of the Microsoft Office suite
• Ability to multitask, prioritize, and manage multiple projects simultaneously
• Excellent written and verbal communication skills and ability to work independently or in a team setting

Our 24/7 On-Call and Triage team responds to Physician Practices, Home Health Agencies, Intellectual Disability Providers, and Assisted Living Providers.
