Requirements
  • Fluency in English and Spanish required.
  • Graduate of an accredited school of nursing.
  • Current, unrestricted LVN or RN license in the state where the program is located.
  • Current Basic Life Support certification; other certifications such as first aid and CPR.
  • Minimum of one year employment experience in public health or a healthcare setting with demonstrated experience in care coordination.
  • Pediatric nursing experience preferred.
  • Pass a pre-employment drug screen and random drug screens throughout employment.
  • Provide proof of work eligibility status upon request.
  • Pass a pre-employment and biennial criminal background checks.
  • Demonstrate the ability to respond sensitively to cultural and socio-economic characteristics.
  • Work collaboratively with staff and professionals.
  • Provide crisis intervention as needed.
  • Ability to work in a fast-paced environment and maintain emotional control.
  • Maintain computer literacy.
  • Work independently and as part of a multidisciplinary team.
  • Demonstrate knowledge of Compass Connections policies and procedures.
  • Provide current physical-medical release and immunization proof (MMR, Varicella, HepB, HepA, Tetanus, PPD; Meningococcal preferred).
  • Must perform physical activity including walking, lifting, bending.

Responsibilities
  • Meet and maintain all federal and state regulatory guidelines and standards applicable to this position.
  • Have extensive knowledge of medical documentation to generate, decipher, and correct for proper documentation methods per federal guidelines.
  • Validate completion of all clinic duties relevant to this position in compliance with federal guidelines and program protocols via daily audits of Portal documentation consistent with the medical clearance process.
  • Perform duties to ensure patient safety by assessing and monitoring health conditions, assist with procedures and operate medical equipment, administer vaccines, medications and treatments ordered by medical provider, monitor for side effects and reactions, and provide teaching patients and staff.
  • Validate systems of records for required facility documentation and reporting to meet Compass Connections, federal and state guidelines within required timeframes.
  • Communicate with Lead Medical Coordinator and Regional Director on policy and procedure issues affecting the quality of health care services on campus.
  • Participate in the development of health and medical related protocols and procedures for federal program.
  • Participate in scheduled meetings, team decisions and operations, including communication of health needs to case managers and clinicians, scheduling health-care appointments, coordinating transportation, ensuring interpretation and translation services, and serving as a liaison for community health-care providers.
  • Maintain complete and confidential health care records and PHI in accordance with privacy requirements.
  • Know and recognize signs of abuse, neglect, exploitation and sexually provocative behavior and implement reporting procedures.
  • Plan, organize, and prioritize daily work using sound judgment.
  • Knowledge of safety protocols including evacuating with children and staff in emergencies.
  • Maintain confidentiality in all areas of service population and program operations.
  • Maintain Compass Connections professional and ethical standards of conduct.
  • Collect, enter, and ensure documentation for medical staff at designated site(s) to meet program guidelines.
  • Supervise and coordinate medical staff to perform clinic duties as directed.
  • Direct and train medical staff at designated sites and coordinate services following policies.
  • Monitor established health care plans for service population within timeframes.
  • Ensure accurate documentation for designated sites in accordance with policies and regulatory requirements.
  • Meet all program and federal partners’ documentation and processes requirements.
  • Other duties as assigned.
Desired Qualifications
  • Bachelor’s degree in relevant field (preferred).
  • Pediatric nursing experience (preferred).
  • Current BLS certification with PALS preferred.
