RATE OF PAY/SALARY: $32.00-$35.00 per hour (based on credentials)

JOB SUMMARY: 

General Purpose: The ideal candidate possesses expertise that qualifies them to perform all nutrition-related duties at the WIC program, including providing and overseeing high risk care participants.

This job currently offers two remote days per week after training is complete. 

Description of Duties:

• Conduct a complete nutrition assessment, determine nutrition risk and enroll participants.
• Conduct a breastfeeding assessment, and provide participant centered counseling that helps a mother initiate breastfeeding, establish a milk supply, exclusively breastfeed and continue breastfeeding.
• Address common breastfeeding concerns and issue breast pumps.
• Document nutrition services provided, including referrals and follow-up to referrals.
• Prescribe, tailor, authorize and reissue food package.
• Provide and document participant-centered nutrition education/counseling (individual/family sessions and facilitated group discussions)for participants
• Ensures referrals for lead testing and immunizations are performed.
• Track participant’s progress, and document outcomes.
• Develop and implement high risk care plans and referral of high risk plans to the Registered Dietitians.
• Coordinates nutrition education, including breastfeeding promotion and support that is responsive to the identified needs/interests of each high-risk participants
• Track high-risk participants’ progress in improving their health and document outcomes.
• Ensure the efficient flow of the assessment and education process.
• Provide leadership in the planning, conducting, and evaluation of Participant-Centered Nutrition Education services.
• Perform other related duties as assigned by the Site Supervisor and Program Director.
• Attend and participate in monthly department, All Staff and other required meetings.
• Be familiar with and adhere to all clinical, administrative, and general policies and procedures as outlined in the Cornerstone Family HealthCare Policy and Procedure Manual and Employee Handbook.
• Maintain confidentiality of all aspects of Cornerstone Family HealthCare including, but not limited to, patient confidentiality, financials, and employee relations.
• Perform other duties as assigned.

Requirements

• Bachelor’s Degree in Nutritional Sciences, community nutrition, clinical dietetics, foods and nutrition from a regionally accredited college or university, or one recognized by New York State Education Department.
