Hoarding Support Supervisor

Pay Range

Base Salary: $53,000- $66,000 (commensurate with experience)

This is a contract position ending January 2027.

Reporting to the Manager, Community Programs and Partnerships, the Supervisor, Toronto Safer Spaces: Hoarding Support leads a specialized staff team that supports vulnerable clients with hoarding challenges that put their housing situation at risk.

The main areas of focus include receiving and processing referrals from Coordinated Access (City of Toronto); conducting initial visits and assessments with all clients and reviewing care plans set by Toronto Hoarding Supervisor/Referral and Coordinated Access/City of Toronto; assigning, scheduling, supporting, and monitoring a team of Mental Health Case Workers specialized at working with clients with Lived experience with Hoarding as well as Modified Cleaners; responding to inquiries regarding VHA Services and providing information and referrals.

The Supervisor will also participate in VHA services and quality initiatives and ensure all activities are conducted within the policies, procedures, and VHA’s standards of confidentiality.

Key Areas of Accountability include:

Client and Staff support

• Responds to all inquiries and provides information on VHA’s services, explains VHA services, and policies.
• Develops a service plan with the client and in collaboration with the client’s care team.
• Ensure that funder targets are met.
• Maintains ongoing communication with client care team throughout service duration and maintains transparency in client’s progress.

Conducting Field Supervision on all clients receiving services

• Schedules initial home visit to review and discuss care plans.
• Develops and discusses discharge planning with clients and/or referral sources.
• Conducts on-site supervision and assessment on all Toronto Safer Spaces: Hoarding Support clients funded through City of Toronto.

Supervision of Hoarding Mental Health Case Workers

• Provides leadership, supervision, training and support for Hoarding Case Workers.
• Ensures quality and performance standards are monitored and adhered to.
• Conducts site visits with staff regularly to ensure service delivery plans are carried out to ensure contract compliance
• Works together with the Manager to develop ideas and recommendations for new programs, enhanced services and productivity improvements.

Contributing to community support teams and service effectiveness

• Participates in identified team meetings with a commitment to contribution and active participation in team and organizational quality improvement processes.
• Participates on project teams as appropriate.
• Understands the financial aspects of care and is accountable for effective, efficient, and responsible use of time and resources when delivering care.

Program Support & Administration

• Works to achieve targets set in relation to Agency and funder objectives.
• Reports to management summarizing intake and case load activity.
• Participates in service evaluation and development.
• Documents trends and service requests to provide input for future planning.

Required Qualifications:

• BSW or MSW Degree in Social Work, Addictions or equivalent combination of education and experience.
• Minimum of 5 years of experience working with clients with complex mental health issues in a community setting
• Minimum 3 years of Supervisory experience
• Experience providing case coordination to vulnerable adults with hoarding, mental health and addictions challenges in home and community setting.
• Proven ability to meet project deliverables and targets within budget an asset.
• Able to provide presentations and training as needed an asset.
• Highly organized and able to efficiently manage time within regular hours of work an asset.
• Knowledge of community and health services and established contacts with other agencies an asset.
• Experience supervising staff within a unionized setting and working knowledge of relevant employment and health and safety legislation and collective agreements, able to communicate in a second language an asset.
• Experience facilitating client support groups is an asset.
• Demonstrated strength in family and individual work and case management an asset.
• Demonstrated ability to assess client needs and formulate care plans an asset.
• Valid driver’s license and use of a car on the job is a requirement.
