Requirements
  • A high school diploma or equivalent, or a General Educational Development credential, is required.
  • Two years of admitting or insurance verification experience in a hospital, health plan, or physician office environment is required.
  • Broad experience in financial counseling and copay collections is required.
  • The candidate must be able to perform mathematical calculations and have extensive experience in a hospital and medical business office setting.
  • The candidate must be able to interpret a patient's insurance coverage, identify services that are not covered benefits, and clearly explain them to patients and providers.
  • Strong problem-solving and customer service skills are required.
  • Knowledge of business office procedures is required.
  • Knowledge of medical terminology and coding is required.
  • Knowledge of grammar, spelling, and punctuation is required to type patient information.
  • The candidate must be able to verify insurance and have advanced knowledge of Current Procedural Terminology codes and medical terminology.
  • The candidate must understand and interpret patient liability and benefits for Health Maintenance Organizations and all payer types.
  • The candidate must be able to read, understand, and follow oral and written instructions and establish and maintain effective working relationships with patients, employees, and the public.
  • Excellent time management, organizational, research and analytical, negotiation, written and verbal communication, and interpersonal skills are required.
  • The candidate must be capable of reading the policy and procedure manual and understanding information pertaining to specific job duties and general information for hospital employees.
  • Fire Life Safety Training from the City of Los Angeles is required within 30 days of hire if the candidate does not have the card upon hire, and it must be renewed before expiration.
Responsibilities
  • Obtain insurance information, verification, and authorization to financially clear patient accounts; update professional and hospital registration systems; select insurance plans correctly in registration and scheduling systems; and use insurer calls or internet portals to obtain and document eligibility and benefits, financial responsibility, and authorization or precertification.
  • Calculate and explain patient liability, including out-of-pocket amounts, coinsurance, and deductibles, and complete calculations for surgery, gastrointestinal services, chemotherapy and infusions, imaging, and invasive and non-invasive procedures according to documentation standards.
  • Contact physician offices when services are denied or redirected or when peer-to-peer review is required, and communicate regarding proposed admissions, special procedures, outpatient referrals, and same-day surgeries.
  • Submit authorizations for surgery, gastrointestinal services, chemotherapy and infusions, imaging, and invasive and non-invasive procedures through the Valor software tool or payer websites, following the applicable protocol; clear assigned worklists in information systems.
  • Enter all authorization information into appropriate registration fields according to documentation standards; submit precertification documentation to third-party payers with correct Current Procedural Terminology and International Classification of Diseases coding; research payer medical-policy requirements; follow up on routine message-center requests within three to five business days; scan authorizations into patient accounts; and document authorization outcomes in the registration system.
  • Perform all other duties as assigned.
  • Ensure insurance eligibility, benefit verification, and authorization processes are completed within insurer time limits to prevent denials or penalties.
  • Document accurate insurance information and authorization details to optimize reimbursement from payers and patients.
  • Maintain working knowledge of insurance plans, contract requirements, and resources for insurance verification and authorization.
  • Run eligibility checks and obtain complete benefit coverage information, including COBRA when applicable, from insurance companies and employers; confirm demographic information; and ensure coordination-of-benefit and insurance-plan codes are accurate.
  • Verify coverage immediately for same-day and next-day inpatient and outpatient add-ons.
  • Determine whether precertification, preauthorization, or referrals are required and obtain them when applicable.
  • Communicate with providers and the team about out-of-network issues, assess contracted and non-contracted payer issues, and document outcomes and next steps.
  • Determine, communicate, and collect patient liability before service and attempt to collect prior balances.
  • Complete the Medicare Secondary Questionnaire accurately with the patient or the patient's representative.
  • Maintain compliance with Health Insurance Portability and Accountability Act regulations as they apply to insurance processes.
  • Attend workshops, in-services, and webinars to remain current on insurance rules, regulations, and industry changes.
  • Submit authorizations for surgery, gastrointestinal services, imaging, chemotherapy, infusions, transplants, and other required services.
