Requirements
  • A bachelor's degree in human services or a related field, or commensurate experience.
  • At least 1 year of case management, direct services, or other related experience.
  • Strong written and oral communication skills.
  • Ability to prioritize competing priorities and make sound judgments.
  • Ability to complete tasks while navigating frequent interruptions.
  • Ability to deliver creative, resourceful solutions to unique challenges.
  • Strong organizational and time-management skills.
  • A valid driver's license and reliable transportation.
  • Ability to drive a 15-passenger van.
  • Willingness to work evenings and weekends when needed.
  • An annual tuberculosis test is required.
  • Ability to sit or stand for long periods.
  • Ability to lift items weighing 10–20 pounds.
Responsibilities
  • Manage a caseload of 10–20 single and family households in supportive housing.
  • Use a strengths-based approach to assess clients' physical health, mental health, social needs, educational needs, life skills, and employment readiness.
  • Use assessment outcomes with clients to determine supports needed to maintain stability and achieve short- and long-term goals.
  • Develop individualized service plans.
  • Assist clients in securing appropriate services and coordinate delivery of employment, housing, financial, mental health, and disability resources.
  • Provide support to clients experiencing crises and implement emergency procedures.
  • Create and maintain financial budgets and provide money-management education; help clients ensure rent and utilities are paid on time.
  • Encourage clients and families to focus on objectives specified in their service plans.
  • Empower and guide clients toward successful completion of the housing program.
  • Assist clients in planning for discharge or graduation from the program when appropriate.
  • Maintain complete client records in accordance with agency best-practice standards.
  • Document all client contact in client files and HMIS, and complete annual reassessments and quarterly reviews.
  • Participate in the on-call rotation.
  • Participate in staff meetings and trainings as directed by the supervisor.
  • Ensure compliance with agency policies.
  • Work as a contributing member of a multidisciplinary team with Shelter House and partner agencies to ensure clients receive needed services in a timely manner.
  • Disseminate relevant client information to appropriate staff members in a timely manner.
  • Use Microsoft 365 applications, including email, calendar, drive, and documents, in daily work.
  • Safely operate Shelter House vehicles to transport clients to various locations.
