Requirements
  • Fluency in English and Spanish is required
  • Graduate of an accredited school of nursing
  • Current, unrestricted LVN or RN license in the state where the program is located
  • Current BLS certification; first aid; CPR
  • Minimum of one year employment experience in public health or a healthcare setting with demonstrated experience in care coordination
  • Pass a pre-employment drug screen and random drug screens throughout employment
  • Provide proof of work eligibility status upon request
  • Pass pre-employment and biennial criminal background checks
  • Ability to respond sensitively to the service population’s cultural and socio-economic characteristics
  • Ability to work collaboratively with other staff members, service providers and professionals
  • Ability to provide crisis intervention according to training
  • Ability to work in a fast-paced environment and maintain emotional control and professional demeanor
  • Maintain computer literacy
  • Work effectively independently and as part of a multidisciplinary team
  • Demonstrate working knowledge of Compass Connections policies and procedures
  • Must provide current physical-medical release and provide proof of immunizations or exemptions for MMR, Varicella, HepB, HepA, Tetanus, PPD; Meningococcal (preferred)
  • Must be able to perform physical activity such as extensive walking, lifting of patients, stretching and bending
  • Must be able to work in a team environment
  • Must maintain professional communication skills (oral, written, listening, non-verbal)
  • Annual influenza vaccination or approved exemption is required as a condition of employment
Responsibilities
  • Meet and maintain all federal and state regulatory guidelines and standards applicable to this position
  • Have extensive knowledge of medical documentation to generate, decipher, and correct for proper documentation methods in accordance with federal guidelines
  • Validate the completion of all clinic duties relevant to this position in compliance with all federal guidelines and program protocols via daily audits of Portal documentation consistent with the medical clearance process
  • Perform duties and tasks to ensure patient safety by assessing and monitoring health conditions, assist with procedures and operate medical equipment, administer vaccines, medications and treatments ordered by medical provider, monitor for side effects and reactions, and provide teaching patients and staff
  • Validate systems of records for required facility documentation and reporting to meet Compass Connections, federal and state guidelines within the required timeframes
  • Effectively communicate, consult, and interact with the Lead Medical Coordinator and Regional Director on policy and procedure issues affecting the quality of health care services on campus
  • Participate in the development of health and medical related protocols and procedures for federal program
  • Participate in scheduled meetings, team decisions and operations, including communication of health needs to case managers and clinicians, scheduling health-care appointments, coordinating transportation, ensuring interpretation/translation services, and serving as a liaison for community health-care providers
  • Maintain complete and confidential health care records and information in keeping with PHI requirements and Compass Connections confidentiality protocol
  • Know and recognize signs of abuse, neglect, exploitation and sexually provocative behavior and implement professional response, reporting and documenting procedures
  • Plan, organize, and prioritize daily work using sound judgment
  • Knowledge of Compass Connections safety protocols including evacuating with children and staff in case of an emergency
  • Maintain confidentiality in all areas of service population and program operations
  • Maintain Compass Connections professional and ethical standards of conduct as outlined in employee handbook including respectful behavior and required dress code
Desired Qualifications
  • Bachelor’s degree in a relevant field
  • Pediatric nursing experience
  • PALS certification preferred
  • Influenza vaccination policy compliance is required (or exemption)
