Requirements
  • The candidate must graduate from an accredited School of Nursing.
  • The candidate must hold current appropriate state licensure and meet the practice requirements in the state of employment.
  • The candidate must hold current or successfully complete CPR Basic Life Support Certification.
  • The candidate must have a minimum of 12 months of experience as a Registered Nurse to receive assigned oversight responsibilities.
  • The candidate must successfully complete all Fresenius Kidney Care education and training requirements for new employees.
  • The candidate must successfully complete three months of experience as a Registered Nurse in home peritoneal dialysis and/or hemodialysis before receiving assigned oversight responsibilities.
  • The position is entry level for Registered Nurses with less than two years of nephrology nursing experience.
  • The position requires the ability to provide direct patient care involving heavy lifting, moving equipment and patients, assisting with ambulation, prolonged standing, bending, and occasionally moving equipment of up to 200 pounds and lifting chemical and water solutions of up to 30 pounds to a height of five feet.
  • The position requires participation in an on-call rotation, including nights, weekends, holidays, or other coverage defined by program needs.
Responsibilities
  • Perform ongoing, systematic collection and analysis of dialysis data for assigned patients, document it in the patient medical record, adjust or modify the treatment plan as indicated, and notify the Supervisor or physician as needed.
  • Assess, collaborate on, and document patients’ and families’ basic learning needs to provide initial and ongoing education.
  • Direct and provide, in collaboration with patients, home care partners, direct and ancillary patient care staff, all aspects of safe and effective dialysis therapy for assigned patients.
  • Administer medications as prescribed or according to approved algorithms, and document medical justification and effectiveness.
  • Initiate or assist with emergency response measures.
  • Serve as a resource for the health care team and participate in staff training and orientation of new staff as assigned.
  • Ensure correct laboratory collection, processing, and shipping procedures and reschedule missed or insufficient laboratory collections.
  • Identify expected outcomes and document and update nursing assessments and plans of care for assigned patients through collaboration with the Interdisciplinary Team.
  • Ensure patient awareness of transplant and treatment modality options.
  • Assist in identifying, evaluating, selecting, and educating Home Dialysis candidates and Home Partners.
  • Assess barriers in Home Dialysis candidates’ home environments and assess partner and family readiness and ability to perform dialysis treatments at home.
  • Train Home Dialysis patients and Home Partners on the safe and effective operation and maintenance of Home Dialysis equipment and treatment supplies through a formal training program.
  • Rotate coverage with other licensed home therapies staff to ensure reliable and adequate coverage.
  • Travel to training sites, other facilities, or patient homes when required.
  • Provide essential functions at other locations, including patients’ homes, when requested.
  • Perform desk work, computer work, and interactions with patients, facility and hospital staff, and physicians.
  • Perform all essential functions under the direction of the Supervisor with guidance from the Educator or Preceptor, or in collaboration with another Registered Nurse.
  • Complete Clinical Advancement Program requirements to advance and perform other duties assigned by the Supervisor.
