Requirements
  • A valid, incumbent nursing license as a Licensed Practical Nurse or Licensed Vocational Nurse is required.
  • A high school diploma or equivalent is required.
  • Current First Aid and Cardiopulmonary Resuscitation license, or the ability to obtain one, is required.
  • At least four years of experience working in geriatric nursing, including experience with medication administration, delivery systems, and pharmacies, is required.
  • The candidate must be able and willing to participate in an on-call rotation.
  • The candidate must meet all mandatory health requirements according to state regulations.
Responsibilities
  • Perform resident assessments and reassessments according to policies and monitor changes in condition.
  • Communicate with the Health and Wellness Director about resident needs to ensure consistent quality of services.
  • Participate in hands-on care, medication administration, and specific nursing tasks when necessary.
  • Schedule and assign caregiving and medication technician shifts to ensure adequate coverage and compliance with staffing guidelines and department allowances.
  • Supervise, train, and support care team members while promoting teamwork and adherence to care standards.
  • Assist with transcribing physicians' orders and reviewing new physicians' orders.
  • Maintain accurate and up-to-date resident records, including care plans, incident reports, and medical documentation.
  • Coordinate care planning with on-site home health agencies within community and company policies.
  • Address residents' special needs, requests, and complaints.
  • Coordinate admissions with the sales team, including supervising move-ins.
  • Understand the community's care regulations and state-specific regulations.
  • Facilitate communication among residents, families, and healthcare providers to ensure seamless care delivery.
Desired Qualifications
  • An associate degree or higher in nursing, or a combination of education and experience, is preferred.
  • Additional coursework in management is an asset.
  • At least one year of relevant supervisory experience, including staff scheduling and management, is preferred.
  • Experience with memory care is a plus.
  • Strong attention to detail and high integrity.
  • Strong communication skills and a teamwork mindset.
  • Strong leadership and team management skills.
  • Strong organizational skills.
  • A positive attitude, empathy, patience, and commitment to treating residents with dignity and respect.
