Responsibilities
  • Performs comprehensive nursing assessment and reassessment including age-appropriate admission and transfer assessments, documenting findings, assessing post-operative or post-invasive procedure patients, and evaluating education and discharge needs of patient and family.
  • Establishes, coordinates and evaluates a plan of care based on analysis of assessment data, patient diagnosis, lab data, tests, physician orders, protocols and standards of care; identifying short and long term goals and formulating nursing interventions to achieve desired patient outcomes; incorporating disease-specific evidence-based practice into the nursing care plan and documentation.
  • Provides and documents nursing interventions based on assessed patient needs, plan of care, and changes in patient status; collaborates with health team members for coordination of daily plan of care; communicates handoff reports; administers and documents medications and IV fluids; completes referrals and requests consultations for special needs; provides patient and family education and discharge planning; clarifies physician orders and implements them; assists physician with procedures; documents readbacks for all telephone/verbal orders; recognizes changes in patient condition and takes appropriate nursing actions; involves family/guardian in care decisions; uses infection control practices in daily care.
  • Documents and/or communicates nursing care and changes in patient condition; performs ongoing evaluation of effectiveness of care and education; evaluates plan of care and modifies as indicated; reports significant changes in clinical parameters to physician; identifies problems, gathers data, suggests solutions, and works toward resolution; reports variation from care/treatment per occurrence reporting policy.
  • Specialized care for high-risk patients including restraint care with alternatives considered, application of restraints per procedure, monitoring and documenting restraint period and care; pain management including assessment, consideration of cultural/spiritual beliefs, prevention-focused techniques, involvement of patient/family; abuse assessment and reporting signs of possible abuse/neglect and action to ensure safety.
  • Demonstrates accountability for professional practice including adherence to quality standards, maintaining current licenses and certifications, staying current with unit information and policies, participating in staff meetings and governance, and adapting to changing workload; coaching and mentoring others; providing handoffs and precise communication; supervision and teaching of students/new staff; maintaining an environment that supports safety and infection control; completing mandatory annual requirements.
  • Ability to relate to coworkers in a professional manner; orienting/precepting new employees; collaborating as a team; acting as resource to patients, families, and staff; delegating and guiding care; communicating plan of care clearly; supervising and teaching students/new staff.
  • Safety/Infection Control: Practices standard precautions, maintains safe environment, hand hygiene, safe patient handling, minimizes noise and adheres to dress code; reports risk management concerns; completes annual mandatory requirements including licenses, certifications, vaccinations, ethics training, safety training, information security, abuse and neglect, bloodborne pathogens, orientation, and competency; reports variations from care per procedures.
  • Service: Provides service excellence including courtesy, meeting physical, spiritual, psychological and emotional needs; effective communication and confidentiality; accurate handoff reports; collaboration and knowledge sharing; cost-effective care and efficiency improvements; adherence to hospital principles and commitments (AIDET) and mission.
  • Incorporates Performance Improvement and Evidenced-Based Practice into practice: helps develop unit goals, remains current with evidences-based initiatives, identifies opportunities for improvement, participates in data collection and performance improvement processes, leads teams when requested, participates in nursing research and quality indicators, contributes to occurrence reviews.
  • Age-Appropriate Care: Provides age-appropriate care with knowledge of normal growth and development, assesses age-specific data, uses age-appropriate communication, involves family in decision-making, provides age-appropriate services.
  • Cost Effective Services: Uses resources efficiently, recommends practice changes to increase efficiency, documents charges for supplies and equipment, prioritizes and completes work on time, uses non-clinical time for audits and mandatory education.
