About the role:

As a Patient Experience Navigator (PEN) for Accompany Health you will:

-Be an advocate for our patients who are often overlooked in our healthcare system

-Support our organizational success metrics by improving and maintaining patient ongoing engagement rates and helping us meeting your quality goals

-Be an early member of our collaborative team providing care coordination to populations with complex healthcare needs

Responsibilities will include:

Manage follow-up appointment scheduling for patients
Help patients schedule and coordinate referrals, imaging, procedures, and screenings appointments and support them in completing these visits
Process prior authorization and DME requests
Organize and locate patient medical records
Support a care pod with daily operational activities including visit planning, virtual visit troubleshooting, and assisting patients with urgent needs
Collaborate with your care pod to carry out patient care plans
Maintain patient confidentiality and abide by HIPAA standards
Other duties as assigned
This role reports to the Manager of Market Operations
Working hours are Monday-Friday 8:00am-5:00pm

Desired skills and experience:

Required
At least 2 years of patient-facing experience in a healthcare setting
At least 1 year of scheduling experience with orders including DME
Previous experience with EMR systems (Athena knowledge is a plus!)
Excellent customer service skills
Ability to quickly build trust with patients
Strong communication skills, and a flexible and positive attitude
Strong computer skills and basic knowledge of G-Suite (e.g., Google Docs)
US work authorization

Preferred
Medical Assistant Certification
