Requirements
  • Pharmacy degree (Pharmacist Doctor of Pharmacy or Bachelor of Pharmacy) from an accredited school of pharmacy
  • Pharmacist license (registered) in Illinois, Missouri, Oklahoma, or Wisconsin (as applicable to the state of work)
  • Resuscitation Certification Requirement: Basic Life Support Healthcare Provider (BLS HCP) required within 90 days of employment
  • No experience required for this position
  • Ability to perform frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs and 25-50 lbs as part of physical requirements
  • Frequent sitting, standing, walking, reaching and repetitive movements; ability to hear alarms and communicate verbally; frequent keyboard use; occasional bending, stooping, kneeling, squatting, twisting and gripping
Responsibilities
  • Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring and dose/therapy adjustment as applicable; documents medication related plans and interventions in electronic medical record
  • Provides clinical consultation delivering best-practice, evidence-based and complete information to practitioners; makes recommendations and encourages safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration
  • Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients; supervises pharmacy technicians and/or interns who support the role of the pharmacist in the drug delivery system
  • Precepts pharmacy students and residents, as applicable
  • Works in a constant state of alertness and safe manner
  • Performs other duties as assigned
