Requirements
  • Two to five years of customer service and/or case management experience, preferably in a healthcare or home health setting.
  • High school diploma or equivalent.
  • Valid New Mexico driver's license, current auto insurance, and a dependable personal vehicle for patient visits.
  • Ability to travel three to four days per week, up to 1,500 miles per month.
  • Computer literacy and comfort using tablets and laptops as primary daily tools.
  • Proficiency in Microsoft Office Suite, including Outlook, Word, Excel, and PowerPoint, with basic spreadsheet and report analysis.
  • Strong written and verbal communication, interpersonal, and presentation skills.
  • Organizational and time management skills sufficient to manage a high-volume caseload.
  • Ability to work independently and efficiently with minimal supervision.
  • Understanding of HIPAA regulations and commitment to maintaining patient confidentiality.
  • Flexible availability outside normal business hours based on patient needs.
  • Successful completion of a mandatory background check through the New Mexico Department of Health.
  • Patient-centered, compassionate, and relationship-driven approach.
  • Ability to advocate for patients in clinical and administrative settings.
  • Ability to collaborate actively within a multidisciplinary care team.
  • Strong reading, writing, and documentation skills.
Responsibilities
  • Serve as the primary liaison between patients, caregivers, clinical staff, and internal departments throughout the care continuum.
  • Conduct home visits to build patient relationships, initiate homecare services, and gather compliance documentation.
  • Attend and advocate for patients at physician appointments, provide program information, and ensure appropriate home care and durable medical equipment arrangements.
  • Manage a caseload of more than 50 patients and collaborate with patients, families, and healthcare providers to oversee medical documentation and renewals.
  • Ensure patients have proper durable medical equipment and provide training to patients, home health aides, and nurses.
  • Conduct weekly, biweekly, and monthly phone check-ins with nurses and home health aides to coordinate patient care.
  • Collaborate with two peer care team members and a lead under the supervision of the Care Team Director.
  • Update and upload documentation in Salesforce and Igea/Axxess electronic medical record systems within 24 hours of home visits.
  • Assist with preparing and processing patient assessments and care plans.
  • Assist with contractor onboarding through the human resources system and ensure compliance documents are completed before start dates.
  • Work and train with onsite nurses on charting and incident reporting, and document incident reports as needed.
  • Pull and review data, reports, and performance metrics to support team goals and quality standards.
  • Provide customer service and build compassionate relationships with patients.
  • Step in for other care team roles as necessary and manage night or weekend on-call duties based on patient needs.
  • Meet established performance metrics.
Desired Qualifications
  • Associate's degree or higher in health administration, social work, or a related field.
  • Experience with Salesforce customer relationship management or a similar platform.
  • Familiarity with the Igea/Axxess electronic medical record system; training is provided.
  • Background in case management, social work, or home health coordination.
  • Knowledge of medical terminology.
  • Experience pulling and interpreting operational reports and performance data.

This transition positions the company for its next phase of disciplined execution and long-term scale. Lew Little will transition from Chief Executive Officer to Executive Chair. In this role, he will remain on the Board of Directors and focus on strategic initiatives, including mergers and acquisitions, partnership development, and Board governance. Lew joined Giving Home in 2022 as the sponsor-appointed CEO and has played a critical role in shaping the organization's current foundation. During his time at Giving Home, he helped break down functional silos, strengthen cross-company collaboration, and invest meaningfully in leadership development - all while reinforcing operational discipline and a patient-first culture. Ben Hanson has been appointed Chief Executive Officer and to the Board of Directors of Giving Home Health Care.
