  • A Bachelor's or Master's degree in Occupational Therapy from an accredited program.
  • An active and valid Occupational Therapist license in New York State.
  • Prior home care experience is not required; new graduates are welcome to apply.
  • Strong interpersonal, communication, and problem-solving skills.
  • Ability to work independently while managing a patient caseload.
  • Reliable transportation to travel to multiple locations in the designated area.
Responsibilities
  • Conduct assessments of patients' physical, cognitive, and emotional needs in their home environment.
  • Develop and implement personalized treatment plans to improve patients' ability to perform daily activities.
  • Work collaboratively with physicians, nurses, home health aides, and other healthcare providers to deliver coordinated care.
  • Educate patients and families on therapeutic techniques, adaptive equipment, and ways to enhance the home environment for safety and independence.
  • Document patient progress, treatments, and goals according to regulatory requirements.
  • Participate in team meetings to discuss patient progress and treatment plans.
