Requirements
  • A Doctor of Pharmacy (PharmD) from an accredited program and an active pharmacy license are required; additional state licensure may be requested.
  • At least 4 years of pediatric pharmacy experience with strong clinical knowledge across disease states is required.
  • The candidate must communicate complex information clearly in virtual settings and demonstrate effective interpersonal communication.

Responsibilities
  • Conduct thorough medication reviews for pediatric patients to ensure appropriateness, safety, and efficacy.
  • Collaborate with healthcare providers to develop and implement comprehensive medication management plans for pediatric patients.
  • Provide remote consultations to caregivers about medication-related concerns.
  • Recommend medication adjustments, therapeutic alternatives, and adherence strategies.
  • Educate pediatric patients and their caregivers about proper medication use, potential side effects, and medication adherence.
  • Provide guidance on lifestyle modifications and other non-pharmacological interventions to optimize patient outcomes.
  • Work with interdisciplinary healthcare teams to ensure seamless communication and coordination of care.
  • Participate in case conferences and team meetings to discuss complex cases and develop integrated care plans.
  • Maintain accurate, timely, and up-to-date documentation of medication management interventions.
  • Ensure compliance with regulatory requirements and organizational policies.
  • Stay current with developments in pediatric pharmacotherapy through ongoing education and training.
  • Contribute to evidence-based guidelines and protocols for pediatric medication management.
  • Participate in quality improvement initiatives to enhance the effectiveness of medication management services.
  • Collect and analyze data to evaluate the impact of interventions on patient outcomes.
Desired Qualifications
  • Board certification in Pediatric Pharmacy (BCPPS) is preferred.
