Requirements
  • One year of experience practicing as a Licensed Practical Nurse.
  • An unrestricted New York State Licensed Practical Nurse license or certification is required.
  • Current CPR and First Aid certification is required.
  • A valid New York State driver's license is required.
Responsibilities
  • Provide quality nursing oversight, link residents to primary care providers and mental health services, and monitor residents' medication compliance daily.
  • Implement medication policies and procedures and ensure compliance with OASAS 820 Regulations.
  • Initiate and coordinate initial referrals with primary medical doctors and mental health providers, and assist when necessary.
  • Assist with follow-up phone calls and advocacy on residents' behalf when necessary.
  • Maintain knowledge of each resident's individualized care plan and provide support according to the care plan.
  • Collaborate with clinical and medical staff in advocating for residents during Medicaid managed care and insurance company reviews.
  • Accurately complete medical documentation for each resident.
  • Provide basic First Aid and CPR when necessary and maintain current certifications.
