Requirements
  • Master's degree in Social Work from an accredited graduate school of social work
  • Two years of experience in a medical setting
  • One year of experience within a cancer setting

Responsibilities
  • Psychosocial Assessment to be completed in a timely manner using theoretical knowledge of human behavior, psychosocial factors having an impact on treatment outcome and understanding of potential ethical/risk issues
  • Clinical intervention including supportive counseling with patients and families, linkage and referral to community agencies and services, facilitating family conferences, helping families cope with difficult decisions (e.g., hospice, DNR, placement), assisting families with communicating their concerns to patients' physicians, collaborating with Case Managers on discharge planning issues, providing group facilitation and program development
  • Triage/Management of clinical caseload including identifying high risk situations, managing caseload, balancing referrals, high risk admissions and ongoing patient care, requesting assistance as needed
  • Legal and ethical standards incorporated into all clinical assessments and interventions (e.g., suspected abuse or neglect, Tarasoff duty to warn, conservatorship, patient rights) and adherence to Tenet Policies, Procedure and Practices in Social Services
  • Documentation including Interdisciplinary Plan of Care, Education Form, Advance Health Care Directive follow-up form and Social Services Progress Notes
  • In service presentations to other hospital staff regarding psychosocial issues of patient care (e.g., Lunch and Learns)
  • Social Service reports including PAS numbers, Case assignment by Social Worker, Absence Approval requests
  • Quality Improvement participation in development, monitoring and analysis of process and outcome indicators for improvement of patient care
  • Maintains competencies for hospital, age-specific and job-specific standards of care
  • Other duties as assigned including Social Work Month activities, cancer-specific support groups and educational programs for patients, families, staff and volunteers
