Requirements
  • Master's degree in Social Work or another directly-related Human Service discipline
  • Current, unrestricted Massachusetts license as an Independent Clinical Social Worker (LICSW) or a Licensed Mental Health Counselor (LMHC) is required
Responsibilities
  • Conduct comprehensive patient assessments by proactively reviewing medical records and performing thorough behavioral health evaluations to identify patient needs, assess for mental health disorders, and evaluate the continuum of substance use from use to dependence
  • Provide evidence-based brief therapeutic interventions including Cognitive Behavioral Therapy, Motivational Interviewing, Problem-Solving Therapy, Supportive Therapy, Behavioral Activation, and Psycho-Education
  • Partner with the primary care team to design and implement person-centered behavioral health treatment plans based on assessments and patient goals
  • Conduct behavioral health screenings to determine appropriate levels of care, identify community resources, and facilitate referrals
  • Develop and deliver educational sessions and curricula on behavioral health and substance use topics for primary care staff and providers
  • Complete clinical documentation meticulously and within expected timeframes, adhering to policies and compliance standards
  • Maintain knowledge of insurance standards for commercial plans and MassHealth products, or quickly learn
  • Actively participate in regular supervision sessions to discuss cases and receive feedback
  • Build and maintain effective working relationships with physicians, patients, visitors, external providers, regulatory bodies, and the broader healthcare community
  • Strictly maintain patient confidentiality and protect sensitive data per privacy regulations and policies
