Requirements
  • Applicants must be certified registered nurse practitioners.
  • Applicants must hold a valid Pennsylvania nursing license.
  • Applicants must participate in continuing education and workshops to maintain a valid license.
  • At least one year of experience working with an elderly community is required.
Responsibilities
  • Perform health histories and in-depth medical examinations for new members to determine preliminary Plans of Care.
  • Initiate, evaluate, and review medical orders and identify opportunities to improve care.
  • Carry out various procedures and document all care and recommendations.
  • Order, interpret, and follow up on laboratory tests and results.
  • Request consultations from in-house and external medical and non-medical providers, including physicians, therapists, dietitians, social workers, and other specialists.
  • Educate staff, members, caregivers, and families on essential care functions to maximize members' independence at home and in centers.
  • Complete ongoing assessments to determine Plan of Care changes or improvements based on member feedback and medical evaluations.
