Requirements
  • Proficiency in Spanish and English is required.
  • Experience in case management or community organizing, preferably within the SFUSD ecosystem.
  • Must have a Bachelor’s degree.
  • Must be a strong, independent worker and demonstrate excellent strategic analysis.
  • Must be comfortable navigating San Francisco neighborhoods for home visits and community outreach.
  • Must have experience working in a culturally and linguistically diverse environment for 2-3 years.
  • Must have experience providing training and/or workshops for adults.
  • Current CPR certification (or attained within 2 months of employment).
  • TB test clearance is mandatory before hire.
  • Some weekends and evenings are required.
Responsibilities
  • Intensive Case Management & Family Advocacy: Direct Intervention: Manage a dedicated caseload of families, providing intensive support to remove barriers to attendance (housing, food security, transportation).
  • Intensive Case Management & Family Advocacy: Home Visits: Conduct home visits for cases of severe absenteeism to build trust and provide direct support.
  • Intensive Case Management & Family Advocacy: Integrated Service Coordination: Connect families with Community Based Organization partners for counseling, food security, and housing stability.
  • Intensive Case Management & Family Advocacy: Mental Health Liaison: Facilitate warm hand-offs to community mental health providers for students and families in crisis.
  • Collaborative Care & Data Strategy: Care Team: Partner with our Care Team under the direction of the School Social Worker to coordinate student mentoring and peer connections/lunch bunches.
  • Collaborative Care & Data Strategy: SST Participation: Serve as a key member of Student Success Teams (SSTs), identifying students who require Tier 2 and Tier 3 attendance interventions.
  • Collaborative Care & Data Strategy: Literacy Integration: Embed student literacy goals within case management plans, ensuring academic success is tied to improved attendance.
  • School Culture, Student Wellness & Student Engagement: Attendance Celebrations: Design and implement monthly school-wide celebrations to highlight perfect and "most improved" attendance with creative incentives.
  • School Culture, Student Wellness & Student Engagement: Peer Connection: Host regular lunch bunches and small-group activities to foster a sense of belonging and positive peer relationships.
  • School Culture, Student Wellness & Student Engagement: Re-engagement: Proactively reach out to and re-integrate students who have become disconnected from the school community or their peers.
  • School Culture, Student Wellness & Student Engagement: Other duties as assigned, supporting overall school operations as needed.
  • Family Education: Workshops: Develop and facilitate three family workshops annually, with a focus on the importance of attendance in the foundational years (TK through 1st Grade)

Mission Graduates helps about 5,000 San Francisco students and their families access higher education through school-based programs, after-school activities, in-school support, and summer initiatives at partner schools. Programs run inside schools with site directors overseeing tutoring, college prep, family engagement, and college-going activities, funded by donations and grants. Exploring careers in community: Mission Housing launches Affordable Housing Youth Tours as part of "The San Francisco Revival" SAN FRANCISCO, Calif. (Oct. 30, 2025) - Mission Housing Development Corporation, in partnership with Mission Graduates, has launched a new series of Affordable Housing Youth Tours as part of The San Francisco Revival - a citywide initiative celebrating the resilience, creativity, and renewal of San Francisco. The first tours kicked off on October 15 and October 22, 2025, with students from John O'Connell High School's Public Service and Construction Pathways, offering a hands-on introduction to the world of community development and affordable housing. The next tour is scheduled for December 3, 2025. During the tours, students visit several Mission District developments - including Valencia Gardens, Alcantara Court, and Avanza 490 - to see firsthand how affordable housing is built, managed, and sustained. Each visit explores not only the architecture and construction, but also the services, partnerships, and people that make these communities thrive. "These tours are about more than just buildings; they're about showing young people that they belong in this work," said Mission Housing Community Engagement Director Chirag Bhakta. "We want the next generation of affordable housing leaders to reflect the same communities we serve."
