Requirements
  • Certified Peer Recovery Specialist/Certified Peer Recovery Specialist (CPRS) in Tennessee. Certification is required.
  • You must be certified in Tennessee and reside in Tennessee for this position.
  • Previous supervisory experience.
  • Minimum of two years of active recovery.
  • Reliable transportation and valid driver’s license.
  • Minimum high school diploma or equivalent.
  • Computer proficiency; must be able to type and navigate a Cloud-based environment and designated teleconferencing application.
Responsibilities
  • Serve as a mentor and role model for individuals at various stages of recovery and provide guidance, encouragement, and support while helping members build healthier, more stable lives.
  • Guide and support Peer Recovery Specialists while ensuring high-quality, person-centered care.
  • Follow the Substance Use Disorder Program guidelines to assess needs, plan care, and complete screenings.
  • Connect members to valuable community resources and services.
  • Collaborate closely with care teams, including primary care providers, managed care organizations, and community partners.
  • Help create a supportive, recovery-focused environment grounded in empathy and respect.
  • Partner with leadership and the enrollment team to grow the census and identify eligible members to enroll in the program.
  • Act as a player/coach for a team of peers while modeling and exemplifying the company core values.
  • Lead and manage a team of peers with the responsibility of overall team success – includes, but is not limited to, driving alignment with operations and clinical team.
  • Manage day to day operations of the peers including regular 1:1s, team meetings, and case conferences.
  • Engage in ongoing performance management and coaching of team members to ensure all issues are addressed and resolved in a timely manner, including but not limited to, disciplinary action as necessary.
  • Maintain a small caseload and provide back up to peers for clinical care during PTO, staff shortages, etc.
  • Conduct meaningful outreach, provide education, and drive member enrollment.
  • Collaborate with member, family and healthcare providers to develop an individualized recovery action plan.
  • Identify and initiate referrals for social service programs – including financial, psycho-social, community and state supportive services.
  • Complete documentation for client engagements, cancellations and care plan.
  • Provide support to clients through a variety of settings and assists with support groups, as needed.
  • Advocate for members and families as needed to ensure the patient’s needs and choices are fully represented and supported by the healthcare team.
  • Represent and promote the Substance Use Disorder Program/Mindoula through participation and interaction with community, professional organizations and community outreach efforts.
  • Achieve set goals; Key Performance Indicators (KPIs).
  • Participate in interdisciplinary team meetings and utilization management rounds and provide information to assist with safe transitions of care.
  • Provide peer recovery support through the on-call crisis line.
  • Promote responsible and ethical stewardship of company resources.
  • Maintains excellent punctuality and attendance during work hours.
  • Other duties as assigned.
