Requirements
  • A Bachelor's Degree in Social Work or a related field.
  • Possess a current social work license as required by the state board where services will be provided: Licensed Social Worker in West Virginia or Ohio, Licensed Bachelor Social Worker in Maryland; no state licensure is required in Pennsylvania.
  • Work prolonged periods standing or sitting.
  • Work flexible hours, including weekends and holidays.
  • Demonstrate excellent verbal and written communication skills.
  • Work collaboratively with health care professionals at all levels to achieve established goals and improve quality outcomes.
  • Possess basic computer knowledge and the ability to operate standard office software.
Responsibilities
  • Complete comprehensive assessments to develop safe, realistic discharge plans of care appropriate for patients, considering psychosocial, emotional, and financial needs, collaborating with the care management team, and documenting in the electronic medical record.
  • Assess changes in the physician's plan of care and their impact on the discharge plan.
  • Review discharge plans on an ongoing basis, communicate changes to the appropriate party, and document the plans in the electronic medical record.
  • Identify potential problems with post-discharge care and initiate early referrals to promote proper utilization of hospital resources and timely transfer to the appropriate level of care.
  • Assist the interdisciplinary team in identifying alternate methods and levels of care when patients do not require acute hospitalization, and take appropriate action to minimize financial loss and improve patient care delivery.
  • Discuss cases with patients and families and consult with physicians as part of care planning and coordination.
  • Collaborate with hospital and community resources to meet defined patient needs and refer patients and families to resources such as income assistance programs, transportation services, and meal assistance.
  • Assess for signs and symptoms of abuse or neglect and make referrals to appropriate agencies, including Adult Protective Services or Child Protective Services.
  • Assist in identifying a medical decision maker, including a Health Care Surrogate, and determine whether a Medical Power of Attorney designee, Living Will, or other advance directive exists.
  • Assist patients in completing Living Will, Advance Directive, and Medical Power of Attorney forms as requested.
  • Work with hospital legal counsel to pursue and expedite guardianship or conservatorship actions when necessary.
  • Provide education and guidance to physicians, patients, families, and other health care professionals regarding Medicare, Medicaid, and other third-party payer coverage issues and regulations.
  • Communicate deviations from expected norms, quality or appropriateness-of-care concerns, length-of-stay issues, and risk-management issues to case-management leadership or designees and appropriate physicians, medical staff directors, or peer review.
Desired Qualifications
  • Certification in Case Management.
  • Two years of social work experience.
