Requirements
  • An advanced practice provider with at least three years of outpatient clinical practice experience, ideally managing hypertension, hyperlipidemia, and primary and secondary prevention of atherosclerotic cardiovascular disease.
  • At least two years of experience concentrated in primary care or a subspecialty with a strong focus on hypertension and lipid management, such as cardiology, nephrology, neurology, endocrinology, primary care, or urgent care.
  • Nurse Practitioners must hold active, good-standing registered nurse and advanced practice registered nurse licenses.
  • Physician Assistants must hold an active, good-standing Physician Assistant license.
  • Maintain required role licensure and certification in good standing and obtain licenses in requested states within 90 days of the hire date; additional state licenses may be required.
  • Work 40 hours per week on Monday through Friday in eight-hour shifts.
Responsibilities
  • Conduct focused and thorough assessments of patients with hypertension, hyperlipidemia, coronary artery disease, diabetes, and other chronic-health conditions, including virtual visits.
  • Formulate diagnoses and individualized treatment plans for chronic medical conditions, including medication management, lifestyle modifications, and monitoring recommendations.
  • Manage a prescription refill inbox in accordance with established protocols and guidelines.
  • Participate in centralized results management workflows by reviewing, triaging, and following up on laboratory and diagnostic results across the patient panel using risk-tiering protocols.
  • Use digital live portal chat and artificial-intelligence-assisted clinical tools, including agent-supported prescribing and inbox triage workflows, for medication renewal, results management, and patient communication.
  • Identify and evaluate risk factors, comorbidities, and potential treatment contraindications.
  • Provide counseling on medication adherence and potential side effects.
  • Collaborate with primary care providers, specialists, pharmacy, clinical nursing, social work, family members, and caregivers to coordinate holistic care and continuity of care.
  • Use the company’s electronic health information system for visits and provide feedback to improve its interface.
  • Maintain accurate and current patient medical records in compliance with applicable legal and ethical guidelines.
  • Participate in quality-improvement initiatives and ongoing professional development related to cardiometabolic care.
  • Comply with applicable laws, regulations, industry standards, patient-privacy requirements, and telehealth regulations.
Desired Qualifications
  • Thorough understanding of cardiometabolic condition management, including disease processes, treatment modalities, medication management, and lifestyle modification for hypertension, hyperlipidemia, coronary artery disease, diabetes, and primary care.
  • Experience working across digital live portal chat and emerging artificial-intelligence-powered clinical tools, with participation in testing, feedback, and iterative improvement.
  • Virtual care experience and interest in continuing clinical nursing practice and conducting virtual visits.
  • Experience with STARS/HEDIS measures, coding clinical comorbidities, and identifying clinical care gaps.
  • Experience or strong interest in centralized results management, live digital or chat-based patient engagement, and artificial-intelligence-enabled clinical workflows.
  • Proficiency in English and Spanish.

Just months after announcing a new round of funding, the Medicare Advantage startup is in talks to raise another round that could value the company at $25 billion, according to sources familiar with the matter. That's a jump from the company's $16 billion valuation in a funding round raised earlier this year, according to sources. Devoted Health, which is based in Waltham, Massachusetts, was founded in 2017 by brothers and former healthcare executives Ed and Todd Park. The startup combines health insurance and medical care for elderly people enrolled in the Medicare Advantage market. The company's proprietary AI tool, Orinoco, helps coordinate medical care for members. Ed Park was previously the COO of athenahealth, and Todd Park was the chief technology officer of the United States in the Obama administration. "We do not comment on our capital raising activity," said a company spokesperson. The fundraising reflects growing investor enthusiasm for healthcare companies that use AI to improve both patient care and the economics of medicine. The back-to-back rounds could also signal investor confidence in Devoted's ability to scale in one of healthcare's largest and fastest-growing markets. Medicare Advantage has become a battleground for both incumbent insurers and venture-backed startups. Medicare Advantage is the private alternative to the traditional Medicare program that provides health coverage to people 65 and older and people with disabilities. The program has grown rapidly over the past decade, with enrollment more than doubling. That boom has attracted both insurance giants and venture-backed startups eager to tap into the hundreds of billions of dollars the federal government spends on the program annually. Devoted generates revenue primarily through government-funded Medicare Advantage premiums, making it part insurer and part healthcare provider. The company had 466,000 members as of January 2026, a year-over-year increase of 121%.
