Requirements
  • A bachelor's degree in social work or human services is required.
  • Must possess any certifications or licenses required by the state of employment to practice in long-term care.
Responsibilities
  • Plan, develop, organize, evaluate, and direct the overall operation of the Social Services department in accordance with the National Association of Social Workers Code of Ethics and federal, state, and local guidelines.
  • Supervise up to two employees, as applicable.
  • Collaborate with social services staff, the interdisciplinary team, and administration to promote and protect patient rights, dignity, independence, and psychological well-being.
  • Maintain working relationships with center employees and meet with administration and nursing staff to collaborate and plan.
  • Ensure that a comprehensive psychosocial assessment is completed for each patient, identifying social, emotional, and psychological needs and strengths.
  • Educate patients and families regarding their rights and responsibilities, health care decision-making and advance directives, effective problem-solving, and available community resources.
  • Provide therapeutic interventions to help patients cope with their transition and adjustment to a nursing center and address behavior or mood problems.
