Requirements
  • A Master's degree is required.
  • A current license in the state of Arizona is required.
  • Basic Life Support certification from the American Heart Association is required upon the date of hire.
  • A Fingerprint Clearance Card application number is required upon hire.
  • A Fingerprint Clearance Card is required within 90 days of hire.
  • The employee must obtain and maintain appropriate clinical competencies for the roles assigned by the departmental leader.
  • Colleagues must understand computers and demonstrate competence using computers and basic software programs.
Responsibilities
  • Assess, treat, plan, implement, educate, communicate, and demonstrate leadership to address the needs of patients and their families.
  • Provide appropriate supervision to supportive personnel, including technicians and students, while assuming responsibility for patient care.
  • Attend and actively participate in patient team conferences and family conferences.
  • Determine the need for follow-up outpatient therapy and make post-discharge recommendations.
  • Document accurately in the electronic medical record and patient chart, maintaining a logical flow between collected data, actions taken, and patient responses.
  • Plan specific measures to meet assessed learning needs and document them in the patient record.
  • Recognize and address the learning needs of patients, families, and other appropriate individuals.
  • Present information clearly and in a manner appropriate to the patient's and family's level of understanding.
  • Implement treatment plans and evaluate and record patient responses to treatment sessions.
  • Participate in developing individual discharge plans responsive to patient and family concerns.
  • Revise and update plans based on assessment.
  • Act as a resource to peers within the rehabilitation department by fielding questions, mentoring, and providing educational opportunities.
  • Act as a resource to other departments and the community by fielding questions, mentoring, and providing educational opportunities.
  • Review the patient's history and appropriate data before initiating an evaluation.
  • Evaluate and treat patients without a physician order or referral as established by the Board of Medical Directors Rule.
  • Assist patients and families in planning for discharge.
  • Report safety-related incidents promptly through the Midas/RDE tool.
  • Attend all safety-related training programs and perform work safely.
  • Monitor the work environment for possible safety issues and ensure others also work safely.
  • Comply with state and federal regulations, statutes, and company policies affecting the employee's area of responsibility.
  • Keep required certifications and licenses current and valid before expiration.
  • Complete mandatory company modules and required job-specific training within the specified time frame.
  • Meet productivity goals according to departmental standards.
  • Attend 75% of department monthly meetings in a 12-month period and remain responsible for all meeting content from the minutes.
  • Respond promptly to new orders in compliance with department policy.
  • Maintain timely and accurate documentation of assessments, interventions, electronic medical record entries, and patient responses.
  • Complete charges in a timely manner and according to proper procedures.
