Requirements
  • Ability to collect patient data including vital signs, height and weight, and food and fluid intake and output
  • Ability to provide personal care including bathing, oral care, skin care, eating and hydration, grooming, dressing, and toileting
  • Ability to assist with turning, positioning, ambulation, and safe patient handling using assistive devices and basic restorative care such as range of motion
  • Ability to report abnormal findings to the registered nurse and participate in care planning and nursing report process
  • Ability to document all care in the patient record
  • Ability to perform point of care testing such as whole blood glucose, blood draw, and electrocardiography
  • Ability to collaborate with materials coordinators for stocking, use, and storage of supplies
  • Ability to maintain a clean, orderly, and safe environment and implement infection control techniques
  • Understanding and incorporation of patient and family rights in decision making about care
  • Awareness of HIPAA and patient privacy requirements and the need-to-know basis for information access
Responsibilities
  • Collect patient data and information relative to health or situation including vital signs, height and weight, and food and fluid intake and output
  • Provide personal care to patients including bathing, oral care, and skin care
  • Assist the patient with eating and hydration, grooming, dressing, and toileting
  • Incorporate safe patient handling into basic restorative care such as ambulation, range of motion, and use of assistive devices
  • Turn and position or mobilize patients as needed
  • Participate in care planning and the nursing report process
  • Provide information to patients and families to participate in decision making about care
  • Document all care in the patient record
  • Promote a clean, orderly, and safe environment and infection control techniques
  • Collaborate with Materials Coordinators for use, stocking, and storage of supplies
  • May serve as activity companion for patients requiring observation
  • Report abnormal findings to RN and patient care team
  • Assist with transitions within the acute care stay toward discharge
  • Provide details related to patient progress toward goals and outcomes to the RN
