Responsibilities
  • Act as the practice-based nurse, supporting on-site providers in facilitating an efficient practice that promotes member engagement, trust, and support
  • Provide care team support from a nursing perspective, and act as a bridge between on-site care and field-based care
  • Coordinate services and relevant Member Action Plan (MAP) goals and tasks both internally and with external providers
  • Review clinical competencies with MAs
  • Assist in rollout and training of new POCT or procedures
  • Assist in population health management for assigned hubs
  • Serve as primary vaccine coordinator for VFC program
  • Oversee pediatric vaccine schedule and administration
  • Provide episodic care management for identified members
  • Serve as liaison with outside lab vendor
  • Collaborate and coordinate member care with specialists
  • Meet with members in the hub as extenders of the primary care providers and other care team members, including medication reconciliation, standing orders, minor procedures, DME coordination, imaging management, prior authorization, and disease management education and oversight
  • Providing in person/telephonic triage for members
  • Evaluate low risk members and assess clinical needs, implementing disease pathways when appropriate to ensure ongoing longitudinal care is achieved
  • Serve as clinical resource for Common Track/low risk population, assisting CHP with clinical goals, education, and/or acute clinical needs
  • Oversee lab compliance
  • Follow-up with members as needed
  • Go above and beyond to connect with patients and partners in a non-judgmental, respectful and empathic manner, to meet their needs, and to provide feedback to the system as a whole as we strive to do better every day
  • Work daily within our custom-built care facilitation platform, to collect data, organize information, track tasks, and communicate with staff, patients, and family
  • Active, unrestricted Registered Nurse license in the state in which you are seeking employment with Cityblock
