- Graduate from an accredited School of Nursing program.
- Hold an unrestricted registered nurse license.
- Be certified as a Nurse Practitioner through an accredited Nurse Practitioner Program with a Master's Degree in Nursing and a concentration and specialty track in Older Adult and/or Family Nurse Practitioner; other concentrations or specialty tracks may be considered based on experience and business need.
- For the Physician Assistant pathway, graduate from an ARC-PA-accredited program, take the Physician Assistant National Certifying Exam, hold state licensure, and maintain National Commission on Certification of Physician Assistants certification.
- Maintain an unrestricted professional license and Drug Enforcement Administration registration in each state of practice where required.
- Work with adult or geriatric populations; two years of experience is preferred rather than required.
- Provide primary and urgent care services to nursing center residents in collaboration with other medical providers in the center.
- Collaborate on-site with the Center Leadership Team on center-specific goals and priorities.
- Collaborate with the Medical Director and/or Attending Physician to provide patient-specific medical care.
- Work collaboratively with Center Leadership to improve patient outcomes through high-quality health care and everyday compassion.
- A minimum of two years of experience working with adult or geriatric populations.
- Concentration or specialty track in an area other than Older Adult or Family Nurse Practitioner, based on experience and business need.
