Requirements
  • Knowledge and skills necessary to provide patient assistance with the referral process.
  • Strong interpersonal skills necessary to interact with patients, families, and clinicians, demonstrate empathy and sensitivity, elicit patient cooperation, and allay apprehensions.
  • Knowledge of community resources and agencies.
  • Basic computer skills including Microsoft Word and Microsoft Excel.
  • Ability to operate a copy machine and fax machine.
  • Basic Life Support certification is required within the first 90 days of hire, although it is not required to start.
Responsibilities
  • Make referrals for patients to other physicians or testing and obtain prior authorizations.
  • Refer patients to other resources as appropriate and coordinate with resources to assure continuity of care.
  • Inform patients of referrals, including the date and time, directions, transportation arrangements when needed, and special preparations or instructions.
  • Fax paperwork needed for referrals.
  • Document referrals in the Electronic Health Record and on the referral log sheet.
  • Document all correspondence with patients in the Electronic Health Record.
  • Maintain a system to track referrals, confirm that patients attended appointments, and ensure referral results are recorded in the patient chart.
  • Use and maintain equipment and supplies properly and practice safety, environmental, and infection control methods.
  • Comply with clinic policies and procedures and requirements from regulatory agencies.
  • Maintain confidentiality of patient information in accordance with organizational policy.
  • Maintain positive working relationships with departments and communicate with departments to resolve operational problems and use resources effectively in delivering quality patient care.
  • Support compliance with applicable federal, state, local, and organizational rules, regulations, protocols, and procedures governing clinical medical services, workplace safety, public health, and confidentiality.
  • Support and participate in continuous quality improvement efforts designed to improve patient outcomes.
  • Consult with clinical teams and clinical support staff to develop and implement policies and procedures that maximize patient-centered communication and services.
  • Report building and equipment problems through appropriate channels.
  • Perform clerical and department-related tasks assigned by the supervisor in a continuously changing medical practice.
  • Attend staff, department, and other required meetings.
Desired Qualifications
  • Bilingual Spanish is preferred but not required.
