Nursing Registered Nurse

Job Description

Case Manager responsibilities include: providing intensive care management for patients, developing individualized care plans and collaborating with an interdisciplinary team, including physicians, to provide a comprehensive approach to care.
Case Managers will also: help minimize and prevent hospital readmission for patients of special needs, guide patients as they navigate the complex world of screenings, treatments, and recovery, follow up with patients and families to ensure their understanding of the plan of care, utilize psycho social expertise to serve each patient's unique needs while ensuring quality and continuity of care and form strong and enduring bonds with patients, families and multidisciplinary colleagues.

Qualifications

Mandatory Requirements:
Current New HampClient or Compact RN License - Required in hand at time of submission
Minimum 2 years current Case Management experience (ACUTE CARE) - Required
Level I or II Trauma Center exeperience - Required
BLS/CPR (AHA) - Required
Current Med/Surg/Surgical/Critical Care/PCU experience - Highly preferred
Prior Travel experience - Highly Preferred
BSN - Preferred
Teaching Hospital experience - Preferred
