IntusCare builds a software platform for Programs of All-Inclusive Care for the Elderly (PACE). It helps healthcare providers manage elderly participants by combining data analytics and predictive modeling to improve outcomes and efficiency. The platform collects and analyzes data from participant care and operations, then offers dashboards and forecasts to guide decisions, risk management, and care coordination. The revenue model is subscription-based, stemming from software licensing and related services. IntusCare differentiates itself by focusing on PACE-specific workflows and tailored predictive tools that support elder-care programs, enabling providers to reduce risks, optimize resources, and improve the quality of care for participants. The company's goal is to make PACE programs safer, more effective, and financially sustainable while helping care teams run their programs more smoothly.

Introducing ILLUMINATE.

Introducing ILLUMINATE. A new amplifier for compliance in PACE. Today, Intus Care is introducing ILLUMINATE, a new service offering built to strengthen PACE compliance. To understand where ILLUMINATE fits, let's first look at the broader IntusCare ecosystem and how it supports the PACE interdisciplinary team (IDT). In the PACE model, the IDT is the light. They bring clinical judgment, coordination, and deep commitment to participants every day. Their work is what makes truly coordinated, community-based care possible. But even the strongest teams can only shine as brightly as the systems around them allow. Administrative burden, fragmented data, and regulatory complexity diffuse focus and pull attention away from proactive care toward documentation gaps, manual workarounds, and retrospective reporting. At IntusCare, its role is to support and extend the work of the IDT by strengthening the foundation that allows teams to fully operate in the way that makes the PACE model of care so effective. An ecosystem centered on the IDT. The IntusCare ecosystem is not new. CareHub EMR, IRIS, and its population health analytics capabilities work together to support 90+ PACE programs nationwide. What is evolving is how Intus Care frame these solutions: as a foundation and a set of amplifiers designed to focus, clarify, and extend the impact of the IDT. At the center is a technology foundation that enables teams to coordinate care, document confidently, and operate fully within the PACE model using a modern, purpose-built system. Surrounding it are amplifiers that sharpen insight, strengthen coordination, and reduce operational distortion. The foundation: CareHub EMR. CareHub is more than a clinical EMR. It is the operational foundation that enables the IDT to function as intended within the PACE model. Built specifically for PACE, CareHub supports interdisciplinary coordination, real-time documentation, compliance-aligned workflows, and defensible decision tracking in a unified environment. Instead of forcing teams into antiquated systems or onto EMRs not built for PACE, it provides the infrastructure needed to deliver coordinated care at scale while staying aligned with regulatory expectations. The amplifiers: IRIS, PRISM, and ILLUMINATE. Where CareHub provides the foundation, its amplifiers extend and focus the impact of your data, decisions, and operations. These solutions combine modern technology with expert services. Each amplifier delivers standalone value while working even more powerfully in combination with CareHub and with one another. IRIS risk adjustment. IRIS is an AI-enabled risk adjustment solution supported by expert services. Like the iris of the eye, IRIS adjusts to changing conditions and protects financial clarity and sustainability. As reimbursement models evolve and regulatory scrutiny increases, IRIS sharpens visibility into participant complexity, ensuring programs are reimbursed accurately and compliantly for the full scope of care delivered. PRISM population health & utilization management. Formerly known as its Population Health & Utilization Management Suite offering, PRISM reflects the evolution of how Intus Care support population health and utilization management in PACE. PRISM pairs the proprietary analytics technology IntusCare was built on with expert guidance to transform fragmented clinical, utilization, and operational data into a clear, actionable view. PRISM reveals cost drivers, utilization patterns, and emerging risk. The result is stronger coordination, better financial stewardship, and more confident decision-making across the organization. ILLUMINATE compliance. ILLUMINATE is its newest amplifier and a service-led offering designed to support sustained compliance, operational consistency, and documentation confidence as regulatory expectations continue to evolve. Introducing ILLUMINATE: compliance that supports the work of the IDT. When PACE compliance is working well, it reinforces care and improves experiences for both participants and the IDT team. It creates clarity, consistency, and defensible documentation that support both participants and the program. PACE organizations are navigating ongoing regulatory updates, evolving oversight expectations, and persistent staff bandwidth constraints. In this environment, small gaps in documentation or inconsistent decision tracking can accumulate over time, increasing operational risk and making audit preparation more difficult than it needs to be. ILLUMINATE shines light into those gaps, bringing structure and visibility so compliance reinforces care instead of competing with it. Intus Care focus on three core pillars: Assess. Establish a clear, objective view of your current compliance and risk landscape so priorities are grounded in evidence, not assumptions. Strengthen. Address gaps early with practical, defensible actions that protect participants, reduce exposure, and reinforce program integrity. Sustain. Maintain alignment over time through ongoing monitoring, structured visibility, and accountability embedded into daily operations. By making the right thing easier to document and track, ILLUMINATE protects your resources, aligns your team, and prevents PACE compliance complexity from pulling focus away from care. Because when the foundation is strong and the signals are clear, the light of the IDT is not diluted by operational or regulatory noise. It is supported and amplified where it matters most: in the care of the participants you serve.
