Requirements
  • Completion of an approved Home Health Aide or Certified Nursing Assistant training program is highly desirable, although equivalent work experience may be accepted in lieu of the certificate.
  • Certification as a Home Health Aide or Nursing Assistant is desired.
  • Reliable transportation is required.
  • One year of experience working with a frail and/or older adult population is required, preferably in a PACE program.
  • Candidates with less than one year of experience must complete the Summit ElderCare training program for health aides before working independently with participants.
  • Health Aides must complete a standardized set of Summit ElderCare competencies annually.
Responsibilities
  • Under the supervision of the registered nurse, assist participants with Activities of Daily Living needs at the PACE center and in participants’ homes and community settings.
  • Contribute to participant care-plan development through interaction with the Interdisciplinary Team.
  • Collaborate with the Interdisciplinary Team to ensure appropriate and continuous care.
  • Carry out non-skilled treatments, including taking vital signs, transfers, toileting, bathing, and dressing.
  • Assist recreational activities and other activities staff with individual and group programs, including planning treatment programs, motivating participation, assisting participants, and evaluating program effectiveness.
  • Assist registered therapists and certified therapy assistants with ordered participant treatments and activities, including positioning, transfers, ambulation, and exercises.
  • Maintain a clean and safe working and living environment in the PACE center and participants’ homes.
  • Assist with meal and snack preparation, serving, feeding as needed, and cleanup.
  • Use safe techniques in all participant interactions.
  • Document accurately and timely in the electronic medical record and other required systems.
  • Participate in daily cleaning and disinfecting of the PACE center in accordance with Centers for Disease Control and Prevention guidelines.
  • Participate in distributing work among health aides to ensure participant care needs and site tasks are completed appropriately and on time.
  • Participate in regular health-aide team meetings at the assigned site.
  • Carry out infection-control precautions and increased cleaning and disinfecting of the PACE center as assigned.
  • Perform duties in accordance with Fallon Health and Summit ElderCare policies and procedures.
