Requirements
  • High school diploma or equivalent education required.
  • A minimum of 1 year of work experience in a medical clinic or similar environment (can include volunteer work, internships, personal caregiving, or formal medical experience)
  • Graduation from a nationally accredited Medical Assistant program (preferred)
  • Experience working with senior populations (preferred)
  • Experience using Electronic Health Records (EHRs) (preferred)
  • Familiarity with common business and communication platforms; naturally adaptable to new digital tools (preferred)
  • Additional healthcare license(s): MA, LPN, CHW, CNA, etc. (preferred)
Responsibilities
  • Conduct warm, supportive, 60-minute sessions in person and virtually to meet each patient’s unique needs.
  • Utilize digital tools to gather data that informs care plans, ensuring each action plan is personalized, accessible, and goal-oriented.
  • Empower patients with age-appropriate action plans, supporting them to make lifestyle changes and celebrating their successes along the way.
  • Manage a caseload of senior patients with respect and sensitivity, fostering a sense of community and connection.
  • Act as a trusted navigator to help seniors make the most of their insurance benefits, providing concierge-level support to access comprehensive care.
  • Build a network of local senior resources to expand available support for patients.
  • Work closely with physicians, nurses, and specialists to ensure a holistic, coordinated care approach for each patient.
  • Act as a compassionate liaison, strengthening communication between patients, families, and healthcare providers to create seamless care experiences.
