Requirements
  • Master's Degree in Social Work or equivalent behavioral health degree eligible for licensure through the CA Board of Behavioral Sciences or approved by program funder (e.g., MSW, MFT)
  • One (1) years of experience in healthcare, case management, homeless services or working with vulnerable populations
  • Current and active licensure or registration with the California Board of Behavioral Sciences (e.g., ACSW, LCSW)
  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Successfully complete Tuberculosis Test, Background Screening, and Drug Test
  • Have reliable transportation
  • A valid driver’s license
  • Proof of insurance and ownership for personal vehicles used during work duties
  • The ability to qualify for PATH's insurance coverage
  • Applicants must be authorized to work for any employer in the U.S.
Responsibilities
  • Perform initial comprehensive biopsychosocial assessment
  • Develop a plan of intervention that addresses identified needs for housing, ongoing case management services, substance abuse treatment, mental health services, and coordination of needed community services
  • Collaborate with the client to develop an individualized treatment plan
  • Provide ongoing case management support to assess progress and ensure treatment plan outcomes are met or adjusted
  • Provide field-based therapeutic supportive case management services addressing mental health, functional, and substance use barriers
  • Consult with LCSW and supervisor to conduct crisis and suicide risk assessment and provide appropriate interventions
  • Complete critical incident reporting as required per contract and PATH policies
