Requirements
  • A minimum of 3 years of clinical registered nurse experience is required.
  • Graduation from a specialty training program in nursing is required.
  • A bachelor's degree in nursing is preferred.
  • A Basic Life Support certification is required.
  • A Pennsylvania registered nurse license is required.
  • Clinical skills, communication, computer literacy, health information documentation, interpersonal communication, patient assessment and care, and teamwork are required.
Responsibilities
  • Serve in an expanded nursing role on an interdisciplinary team to promote preventive patient care, offer screening services, and coordinate disease management services.
  • Provide primary clinical support to providers, patients, and families to assist with diagnosis and treatment.
  • Triage inbox messages and manage patient interactions with the clinic by identifying patient needs, intervention pathways, clinical guidelines, and diagnostic tests to develop individualized care plans.
  • Assess the healthcare, educational, and psychosocial needs of patients and families and involve them in treatment planning based on realistic goals and interventions.
  • Investigate healthcare options and facilitate communication among patients, primary care providers, and other healthcare team members to eliminate barriers and identify treatment interventions.
  • Educate patients, families, and healthcare team members on care coordination, disease management, case management programs, and outcomes.
  • Serve as a disease-management resource for patients, families, and healthcare team members.
  • Implement clinical interventions based on risk stratification and evidence-based clinical guidelines.
  • Collaborate with primary care providers to improve adherence to evidence-based clinical guidelines and promote best practices by initiating and adjusting therapies as directed by practitioners, with appropriate follow-up and monitoring.
  • Coordinate laboratory and diagnostic tests for disease management programs.
  • Maintain required documentation in EPIC for disease management activities.
  • Collect and enter data and patient information according to department standards.
  • Participate in professional and nursing continuing education programs and pursue professional development opportunities.
  • Review current literature on effective teaching, learning, and disease-management strategies and incorporate appropriate techniques into practice.
  • Maintain active involvement in post-discharge transitions-of-care coordination.
  • Assist leadership in designing, implementing, and evaluating care coordination programs supporting organizational goals and strategic business strategies.
  • Collect and prepare laboratory specimens and complete associated paperwork to expedite laboratory studies accurately and efficiently.
  • Insert, monitor, regulate, and discontinue intravenous lines as required by providers and within the scope of practice.
  • Administer medications and vaccines, including intradermal and skin testing, under provider direction and within the scope of practice.
