Requirements
  • Graduated from an accredited nursing program within the past 12 months.
  • Obtain a nursing license and registration within the timeline defined by state regulations or within 90 days of hire, whichever occurs first.
  • Hold a current appropriate state Temporary Permit.
  • Meet the practice requirements in the state of employment.
  • Maintain required credentials and certifications in active and valid status, including timely renewals.
  • Handle demanding situations, multitask, and remain flexible to changing needs.
  • Maintain regular and punctual attendance.
  • Provide direct patient care involving heavy lifting, moving patients, assisting with ambulation, prolonged standing, bending, and occasional movement of machines and equipment up to 200 pounds with assistance.
  • Pass the Ishihara Color Blindness test as a condition of employment; reasonable accommodation may be considered.
Responsibilities
  • Direct Patient Care Technicians and delegate tasks to direct patient-care staff while monitoring safe and effective care delivery.
  • Assess daily patient-care needs and develop appropriate patient-care assignments.
  • Monitor staff techniques and adherence to facility policies and procedures.
  • Participate in staff training, new-staff orientation, and required staff meetings.
  • Educate patients and families about end-stage renal disease, renal disease, vascular access, dialysis therapy, diet and fluid management, medications, laboratory values, transplant awareness, and treatment adherence.
  • Provide safe and effective hemodialysis care to patients with end-stage renal disease.
  • Implement treatment prescriptions, including sodium and ultrafiltration modeling where appropriate.
  • Assess patient responses to hemodialysis, adjust treatment plans as indicated, and communicate concerns to the Team Leader or physician.
  • Identify and communicate patient-related issues to the Team Leader or physician.
  • Initiate nursing assessments and conduct ongoing evaluation and documentation of patient-care needs.
  • Participate in pre-evaluation, initiation, monitoring, termination, access homeostasis, and post-evaluation of hemodialysis treatments.
  • Intervene when patient adequacy status changes and troubleshoot access-flow issues identified by Online Clearance Monitoring or the Adequacy Monitoring Program.
  • Provide, supervise when applicable, and monitor hemodialysis access care.
  • Administer and document prescribed interdialytic transfusions and notify the physician and blood supplier of adverse reactions.
  • Ensure accurate and complete Patient Care Technician documentation on Hemodialysis Treatment Sheets.
  • Review, transcribe, and enter physician laboratory orders into the Medical Information System.
  • Prepare laboratory requisitions, ensure correct specimen tubes and procedures, and follow up on missed or insufficient specimens.
  • Package and deliver specimens according to Department of Transportation policies and laboratory requirements.
  • Report alert, panic, and abnormal laboratory results to the appropriate physician and forward results as requested.
  • Enforce company policies, procedures, and applicable state and federal regulations.
  • Maintain safe, efficient, and effective shift operations and act as a resource for staff members.
  • Meet with the Clinical Manager regarding personnel and patient-care status, issues, and information.
  • Collaborate with physicians and healthcare team members to interpret, adjust, and coordinate patient care.
  • Assist patients with prescription refills according to company policies.
  • Enter physician orders and patient information into the Medical Information System and oversee documentation accuracy.
  • Maintain the facility drug list and ensure adequate emergency equipment.
  • Maintain competency in emergency procedures and initiate cardiopulmonary resuscitation and emergency measures during cardiac or respiratory arrest.
  • Ensure appropriate vaccinations, immunizations, and annual tuberculosis testing.
  • Administer prescribed and as-needed medications, document effectiveness and medical justification, and maintain controlled-substance records.
  • Assist with coordinating patient transportation when necessary.
  • Maintain a clean, safe, and sanitary dialysis treatment area.
  • Operate dialysis-related equipment safely and effectively.
  • Ensure dialysis stations, machines, and chairs are clean and properly positioned.
  • Address blood spills according to bloodborne-pathogen control policies.
  • Implement patient admission, transfer, and discharge procedures.
  • Review daily Hemodialysis Treatment Sheets and ensure their completion.
  • Follow opening and closing procedures and monitor that staff and patients safely leave the premises.
  • Initiate, document, and complete Continuous Quality Improvement activities and monthly reports.
  • Complete monthly nurses' progress notes and maintain complete, accurate patient medical records.
  • Review transplant status and follow established procedures.
  • Complete patient care plans for new or unstable patients.
  • Complete long-term programs, annual nursing histories and physical assessments, annual standing-order reviews, and other assigned duties.
