Requirements
  • An Associate of Arts degree or diploma from a School of Nursing is required.
  • At least 12 months of hemodialysis experience is required; 18 months is required in Texas.
  • A valid state nursing license is required.
  • Current CNN or CDN certification is required, or it must be obtained within one year of hire.
  • Current Basic Life Support certification is required.
  • A current driver's license is required.
  • The ability to assess patient status and take appropriate action is required.
  • The ability to function at a high level with little operational oversight is required.
  • The ability to communicate verbally and in writing with diverse backgrounds and organizational levels is required.
  • The ability to define problems and present solutions is required.
  • The ability to sit and work at a computer for extended periods is required.
  • The ability to walk, sit, and lift no more than 50 pounds is required.
  • The ability to identify and differentiate colors is required.
  • Willingness to travel to network facilities and sites as needed is required.
Responsibilities
  • Coordinate and provide nursing care, training, and education for dialysis patients.
  • Collect, analyze, and accurately record relevant patient health data.
  • Analyze assessment data to identify patient problems.
  • Develop plans of care describing interventions to attain expected outcomes.
  • Implement and record interventions in the electronic medical record to attain expected outcomes.
  • Evaluate patient progress toward identified goals.
  • Adhere to approved, research-based practice standards.
  • Direct and supervise patient care provided by LPN/LVNs and support personnel.
  • Use equipment and materials safely and cost-effectively.
  • Perform and document point-of-care testing and quality control according to established laboratory policies and procedures.
  • Function as a patient advocate and ensure care is delivered in a non-judgmental and non-discriminatory manner.
  • Ensure patients are informed about consent requirements and have signed for indicated procedures.
  • Participate as an active member of the department performance improvement committee.
  • Track quality assurance performance improvement activities and develop action plans to meet goals; meet with the Nurse Manager as needed to drive quality assurance performance improvement goals.
  • Use current data and research so the home program reflects industry trends that improve patient outcomes.
  • Work collaboratively with the interdisciplinary team, other departments, and outside institutions.
  • Work collaboratively with team members to maintain inventory.
  • Support dialysis teaching days and assist with planning.
  • Provide support for home patients and family members while maintaining professional boundaries.
  • Provide direct patient care as needed in response to on-call issues.
  • Collaborate with physicians and staff on patient care as needed.
  • Maintain clinical staff competencies.
  • Ensure all required regulatory training is attained annually.
  • Perform similar or related duties as requested or directed.
