Requirements
  • A bachelor's degree in Business Administration, Healthcare Administration, Public Health, Marketing, or a related field, or an equivalent combination of education and relevant leadership experience.
  • At least three years of experience in healthcare sales, outreach, enrollment, business development, care navigation, social services, community engagement, or a related healthcare environment.
  • At least one year of experience leading, coaching, mentoring, or developing individuals or teams, with demonstrated success in driving performance, developing talent, and achieving business objectives.
  • Experience working with older adults, frail elderly populations, caregivers, or within PACE, Medicare, Medicaid, managed care, or other value-based healthcare environments.
  • Ability to develop and execute growth strategies while managing operational performance and achieving organizational goals.
  • Strong leadership, coaching, and talent development skills to build engaged, high-performing teams and foster accountability, collaboration, and continuous improvement.
  • Strong communication, relationship-building, and collaboration skills to engage team members, participants, caregivers, providers, community partners, and organizational leaders.
  • Ability to analyze operational and performance data to inform decisions, prioritize initiatives, and improve organizational outcomes.
  • Experience using Microsoft Office, including Excel, and ability to learn customer relationship management, electronic medical record, and participant relationship management systems.
  • Ability to manage competing priorities, navigate ambiguity, and lead effectively in a fast-paced, evolving environment.
  • Valid driver's license, reliable personal transportation, and automobile insurance meeting applicable state requirements, with ability to travel throughout the service area.

Responsibilities
  • Lead, coach, and develop the Outreach and Enrollment team by fostering accountability, professional growth, continuous learning, and belonging.
  • Develop and execute outreach and enrollment strategies that drive participant growth, strengthen referral partnerships, and support organizational objectives.
  • Monitor team performance, operational metrics, and enrollment outcomes; use data and insights to identify opportunities, improve performance, and remove operational barriers.
  • Partner with center leadership and cross-functional teams to deliver a coordinated, participant-centered enrollment experience and support successful transitions into the PACE program.
  • Provide leadership and guidance on complex enrollment cases, participant eligibility, family dynamics, and operational challenges.
  • Build and maintain strategic relationships with healthcare providers, community organizations, referral partners, and other external stakeholders.
  • Lead community outreach initiatives, educational events, and local growth activities to increase awareness of the PACE program and support market growth.
  • Ensure compliance with federal and state regulatory requirements and company policies through operational oversight, quality monitoring, and continuous coaching.
  • Drive continuous improvement by identifying best practices, streamlining workflows, and implementing solutions that improve participant experience, operational efficiency, and team performance.
  • Communicate with participants, caregivers, team members, and stakeholders in person, by phone, and through electronic systems.
  • Travel between work sites or community locations as needed.
  • Follow safety procedures and infection-control protocols and use personal protective equipment when required.
  • Perform physical tasks that may involve bending, reaching, lifting, assisting participants, and lifting, carrying, pushing, or pulling supplies, equipment, or materials typically weighing up to 25–50 pounds with assistance or mechanical aids.
  • Perform other related duties as assigned.
Desired Qualifications
  • Experience leading outreach, enrollment, intake, or growth teams within PACE, Medicare, Medicaid, managed care, or other value-based healthcare environments.
  • Knowledge of PACE, the Centers for Medicare & Medicaid Services, the California Department of Health Care Services, and applicable federal and state regulatory requirements related to outreach, participant eligibility, enrollment, and care coordination.
  • Experience developing strategic community partnerships and provider referral networks that support sustainable organizational growth.
  • Bilingual proficiency in Spanish, Mandarin, or Cantonese.

Habitat Health holds ribbon cutting for South Los Angeles PACE Center. February 27, 2026 Habitat Health, in collaboration with Kaiser Permanente, officially celebrated the opening of its South Los Angeles Program of All-Inclusive Care for the Elderly (PACE) Center today with a ribbon-cutting ceremony attended by community leaders and healthcare partners. The event marked a significant milestone in expanding access to coordinated, whole-person care for older adults age 55 and over in one of Los Angeles County's most medically underserved regions. "Today is more than the opening of a building. It marks a new chapter in how we care for older adults in South Los Angeles," said Sophia Guel-Valenzuela, Executive Director of Habitat Health South Los Angeles. "For too long, many older adults and their caregivers in this community have faced barriers to coordinated outpatient care. This center was built with those realities in mind. We are honored to be another PACE access point providing comprehensive, culturally responsive care directly for the neighborhoods that need it most." South Los Angeles experiences disproportionately high rates of chronic conditions such as diabetes, asthma, heart disease, and hypertension. Adults in the region face significantly higher rates of preventable hospitalizations and emergency department visits compared to other parts of the county. In fact, diabetes-related hospitalizations in South Los Angeles are 2.5 times higher than in West Los Angeles, highlighting serious gaps in preventive and coordinated care. "Opening the doors to this Habitat Health PACE center means opening doors to longevity, connection, and health for the community of South Los Angeles. Habitat Health's patients will have access to Kaiser Permanente's model, which is grounded in providing the right care, at the right place, at the right time - whether that's inside of our clinic walls or beyond. We're proud to play a key role in keeping this community healthy, independent, and active," said Dr. Bechara Choucair, chief health officer, executive vice president, Kaiser Permanente, and a Habitat Health board member. Habitat Health is both a PACE health plan and a direct care provider, offering integrated medical and social services under one roof. Services include primary and specialty care coordination, social services, day center activities, transportation, physical therapy, prescription management, nutrition counseling, home care coordination, dental services, and access to diagnostic and specialty services. Through its collaboration with Kaiser Permanente, participants also receive coordinated specialty and hospital-based care through Kaiser Permanente's regional network.
