Requirements
  • High School Diploma or equivalent.
  • Proficiency in Microsoft Office Suite, including Outlook, Teams, Word, Excel, PowerPoint, and OneDrive.
  • Case management or housing navigation experience serving homeless populations or similar populations.
  • Customer and client relation skills.
  • A valid California driver's license and eligibility for company vehicle insurance.
  • Possession of a personal vehicle.
  • Proficiency in Microsoft Outlook, SharePoint, Teams, and Calendar.
  • Ability to read and comprehend a lease agreement.
  • Ability to maintain confidentiality of client and work-related information.
  • Understanding of housing tenancy and sustainability services.
  • Understanding of the social service landscape and resources available to clients.
  • Familiarity with harm reduction principles, trauma-informed care, housing resources, and health care delivery systems such as the California Department of Health Care Services.
  • Ability to meet deadlines and performance expectations and work under pressure.
  • Ability to work independently and within a team environment.
Responsibilities
  • Maintain a maximum caseload of 30-35 clients.
  • Network and collaborate with relevant internal and external personnel to promote housing retention outcomes.
  • Advocate for clients and connect them with resources supporting stabilization and housing retention, including financial benefits, mental health, substance abuse, hygiene, shelter, medical treatment, and legal advocacy resources.
  • Attend Interdisciplinary Care Team meetings when scheduled and provide updates to health care networks on mutual clients and CalAIM programs.
  • Coordinate, prepare, and maintain required charting and documentation in a timely and thorough manner.
  • Maintain client records according to HIPAA and client confidentiality requirements.
  • Conduct comprehensive assessments to identify clients' housing barriers and needs.
  • Monitor client progress regularly and adjust support as needed.
  • Act as a liaison between clients and landlords to address concerns and potential conflicts.
  • Advocate for clients in lease disputes and negotiate payment plans or other accommodations when necessary.
  • Provide training on tenant rights, household maintenance, conflict resolution, and other essential life skills.
  • Connect clients with community resources for mental health services, substance abuse treatment, and employment opportunities.
  • Collaborate with members of each client's care team to address client needs.
  • Attempt to meet all clients in person each month and use at least three communication methods monthly when a client cannot be contacted.
  • Develop customized sustainability plans with measurable goals, actionable steps, timelines, and objectives, and support clients in achieving them.
  • Maintain detailed client records, including case notes, progress updates, and communication logs.
  • Disenroll a client from services after four unsuccessful contact attempts through electronic, telephone, and written methods, when appropriate.
  • Assist clients with the recertification process when they qualify for a public housing authority opportunity.
  • Assist clients with timely monthly rent payments and coordinate their housing experiences.
  • Meet with clients at their residences in accordance with role expectations.
  • Complete training and development opportunities assigned by the supervisor.

Illumination Health + Home creates joyful Carnival for families experiencing homelessness. Annual Carnival for Kids brought rides, entertainment, and connection to Orange County. SANTA ANA, Calif., July 16, 2026 (GLOBE NEWSWIRE) - Illumination Health + Home (IHH), an Orange County-based nonprofit disrupting the cycle of homelessness through integrated housing and healthcare services, hosted its annual Carnival for Kids on July 11, bringing together 400+ guests for a day of family-friendly fun. The free community event celebrates IHH's Children and Families Program, which provides emergency housing, recuperative care and trauma-informed services to families experiencing homelessness. Guests gathered at the Heritage Museum of Orange County for carnival rides, live music, clowns, magicians and food from Chef Troy Hashimoto of Branded by Compassion. IHH leaders and community partners - including CalOptima Health CEO Michael Hunn, Santa Ana Mayor Pro Tem David Penaloza, Santa Ana Mayor Pro Tem David Penaloza, and a representative from the Office of Congressman Lou Correa - offered remarks and presented IHH's 2026 Kids Champion Award, honoring an organization that has made a lasting difference for families experiencing homelessness in Orange County, to School on Wheels. "Carnival for Kids is a reminder that community and connection are essential to ending homelessness," said Dr. Pooja Bhalla, CEO of IHH. "When the community comes together, it gives people a sense of normalcy, and it allows us to keep expanding access to the housing, health care and critical services they need to rebuild their lives." The carnival reflects IHH's broader commitment to meeting families where they are. That same approach guides the Children and Families Program, which includes five Family Emergency Homes and one Children and Family Recuperative Care program for children with medical needs. Services include developmental and behavioral health screenings, counseling, connections to medical and behavioral health care, parenting education, and educational support - helping families address immediate needs while building toward stability and permanent housing. School on Wheels, founded in 1993, provides free one-on-one tutoring and mentoring to K-12 students experiencing homelessness across Southern California, working alongside families in shelters, motels, and temporary housing to keep children on track in school. "Education is one of the most powerful tools a child can have, and for students experiencing homelessness, support can change the course of their lives," said Maegan Mattock, Chief Regional Officer of School on Wheels.
