Requirements
  • Holds a valid and unencumbered license or verification to practice as a licensed practical/vocational nurse in the state where services are provided.
  • Has reliable transportation.
  • Provides a copy of current tuberculosis results or a chest X-ray.
  • Provides a copy of current cardiopulmonary resuscitation certification according to agency policy.
  • Can be flexible and organized and exercise independent, sound judgment to perform the job duties and responsibilities.
  • Can regularly sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull, reach with the hands and arms, and perform other physical activities required by the job duties.
  • Has vision and hearing capabilities within normal limits, with or without aid.
  • Can frequently lift or move up to 15 pounds and occasionally lift or move 100 pounds or more if applicable.
  • Can perform the anticipated level of physical assistance required to provide safe client care.
Responsibilities
  • Provides skilled nursing as ordered on the plan of treatment and nursing care plan.
  • Documents each shift visit to reflect skilled nursing care rendered in accordance with physician orders.
  • Identifies new problems, changes, and resolutions and follows through by notifying the director or supervisor, notifying the physician to obtain necessary intervention orders under state regulations, and documenting the information in clinical notes, supplemental physician orders, and the nursing care plan.
  • Uses aseptic technique as required.
  • Performs specific treatments and administers medications according to the care plan.
  • Functions as a team member under the direction of the registered nurse.
  • Works shifts according to the order on the plan of treatment.
  • Communicates skilled observations and significant changes in patient status to the Director of Clinical Services or Clinical Manager.
  • Requests order modifications or additional orders based on the patient's clinical status and skilled nursing judgment.
  • Documents physician collaboration in the medical record.
  • Demonstrates and documents appropriate use of available services.
  • Participates in other agency-mandated activities, such as in-services, as requested by the Agency Director.
  • Adheres to agency policies, procedures, and handbook requirements.
  • Completes all clinical documentation accurately and submits it on time according to agency policies and guidelines.
  • Maintains a positive and professional presence in the company and community.
  • Adheres to the dress code with a clean and neat professional appearance.
  • Reports on time and as scheduled to complete work within the designated time.
  • Provides care in private homes and travels frequently to complete home health visits.
