Requirements

Responsibilities
  • Assists in meeting the psychosocial needs of patients and families through assessment of inpatients based on assignment; patients are screened for continuing care needs including emotional support, community resource needs, home care, in patient care and health insurance issues; the Case Manager will assess patients and conduct medical insurance reviews for them; educates, coordinates, and collaborates with nurses, physicians, Continuing Care Coordinators and interdisciplinary members of the Healthcare team to assure an ongoing comprehensive discharge plan
  • Ensures appropriate use of resources within an appropriate length of stay; collaborates with the multidisciplinary team on the patient’s treatment plan
  • Reviews admissions and continued stays in accordance with established criteria; conducts all adjunct procedures such as referrals to the Medical Director, keeps statistics, performs insurance reviews, and identifies quality of care issue
  • Utilizes electronic Utilization Management platform for assigned units/patients and assesses the patient’s medical, social, financial, and psychological status within three days of patient’s admission; manages discharge planning for patients in Intensive Care Unit and ensures discharge plan until transfer to another level of care; consults with Continuing Care Coordinator for discharge planning needs for patients on palliation, substance abuse, homelessness, uninsured, victims of domestic violence, elder abuse, and those needing inpatient/outpatient terminal care, inpatient, outpatient psychiatric care, Subacute and Acute Rehabilitation placements, Home Visiting Nurse services and patients in assisted living facilities, group homes and adult homes; dedicated CCC consulted for patients newly diagnosed with cancer for community support services; CCC consulted for patients requiring guardianship and follows through on discharge plan; Case Manager assigned to Emergency Department and will consult CCC for social needs
  • Counsels patient and significant others in admitting criteria and referral process to services; follows through on referrals to ensure safe discharge; completes Patient Rights and Important Message from Medicare documents during regular hours and follows through with IPRO if patient requests an appeal; refers cases to the CCC and assists as needed such as Visiting Nurse Services, Inpatient Skilled Nursing Care and Acute Rehabilitation, Long Term Home Care, Personal Care Services, Home Infusion Therapy and Wound Care, Durable Medical Equipment, and other community resources
  • Counsels patients and families regarding anxieties and stress related to illness and hospitalization, coping with residual disability, fears of helplessness, loss of capabilities, and death
  • Collaborates with community professionals to develop discharge plan and continuity of care; maintains resource file and follows referral procedures to extended services; uses interview skills to determine patient and family discharge planning goals and incorporates these into discharge plan; coordinates multi-disciplinary team goals
  • Assists in obtaining medical doctor orders and insurance authorization for post-hospital needs such as Certified Home Health Agency, Inpatient Skilled Nursing Care and Acute Rehabilitation, Infusion therapy and Wound care, Medical supplies and equipment, Transportation
  • Communicates to patients their right to seek services outside of insurance authorization if they will assume payment; Assists family members in arranging burial and contacting clergy and family
  • Pre assessment, initial discharge planning and overview of community and residential services for Same Day Surgery patients preparing for total joint replacement; assigned to Ambulatory Services Unit for discharge planning needs
  • Collaborates with hospital benefits area in identifying changes of benefit status or lack of insurance
  • Participates in performance improvement activities by monitoring length of stay and denials; refers cases regarding quality of care and utilization issues to appropriate administrator
  • Performs medical insurance reviews concurrently and retrospectively; collaborates with physician and nursing to provide medical necessity criteria to payor; assists in appeals by documenting crucial areas during stay
  • Assumes responsibilities of Case Coordination Center when CCC staff is limited
  • Assumes other responsibilities when assigned
Desired Qualifications
  • Bachelor’s degree preferred
  • New York State PRI assessment certification desirable
  • Three years discharge planning or case management experience preferred
  • Ability to manage complex discharge planning needs; ability to coordinate with physicians and social workers; written and verbal communication skills; fluency in English; knowledge of community resources; competency with computer systems and electronic health records; knowledge of Medicare and Medicaid regulations and utilization review processes
  • Spanish language ability or other community dominant language ability an asset

St. John's Riverside Hospital St. John's Riverside Hospital, a leading community-based healthcare provider in Yonkers, New York, recently experienced a data breach that potentially exposed personally identifiable information (PII) and protected health information (PHI) of at least 2,238 individuals across the U.S. According to a disclosure filed with the U.S. Department of Health and Human Services on Nov. 14, 2025, the breach originated from unauthorized access to a limited number of employee email accounts. The incident was discovered in September 2025, when the hospital detected suspicious activity involving the distribution of phishing emails and an attempt to reroute payment funds. The compromised email accounts contained a range of sensitive information. For some individuals, the exposed data included personally identifiable information (PII) such as name, date of birth, Social Security number, driver's license or state identification number and financial account number. For others, protected health information (PHI) was involved, including health insurance details, medical condition information, treatment provider name, medical record number, treatment cost information and diagnosis or treatment information. St. John's Riverside Hospital's response. Upon discovering the breach, St. John's Riverside Hospital immediately took steps to secure its systems. The hospital changed passwords, revoked session tokens, reset multifactor authentication and engaged data security and privacy professionals to investigate the incident. The unauthorized activity was contained and remediated, and the hospital conducted a thorough analysis to identify affected individuals.
