Requirements

Responsibilities
  • Establish contact with newly diagnosed cancer patients and/or family members or caregivers to guide them through the disease-specific care system.
  • Understand the clinical pathways established by the disease-specific systems and direct patients to health care services within the organization for timely treatment and survivorship.
  • Identify and address barriers to care that might prevent timely and appropriate treatment for cancer diagnosis.
  • Connect patients with resources, health care and support services in their communities and assist the patient and disease-specific physicians in the transition from active treatment to survivorship.
  • Maintain records of all patient-related activities.
  • Develop clinical pathways for appropriate care in cooperation with disease-specific clinical teams.
  • Act as the contact clinical person in resolving all patient-related concerns.
  • Ensure all medical information has been received by referring physicians and review all medical information prior to patient visits; discuss with disease-specific physician prior to patient visit.
  • Maintain contact with referring physicians to ensure they are up to date on the patient plan of care.
  • Collaborate with the research department to develop outcomes measurement tools.
  • Assist in identifying any open trial for patient eligibility.
  • Provide safe patient care by demonstrating organizational skills to coordinate delivery of quality care for assigned patients/families.
  • Develop a culturally competent plan of care identifying patient problems, expected outcomes, and preventative measures.
  • Perform systematic patient and family assessment relevant to practice settings.
  • Deliver care that is age-appropriate.
  • Evaluate effectiveness of care and adapt plan based on patient/family response.
  • Establish professional relationships with nurses, physicians, and other colleagues and patients and families.
  • Educate patient and family about illness and treatment including preventative measures; support community-based activities within and beyond Lahey Clinic.
  • Demonstrate professional behavior by adhering to unit policies and procedures; practice guidelines; ANA Code of Ethics; be a responsive team member; fulfill role as teacher; ensure regulatory compliance and readiness.
  • Uphold standards of quality nursing care; commit to lifelong learning; demonstrate evidence-based practice in nursing.
  • Incorporate Lahey Clinic Guiding Principles, Mission Statement, and Goals into daily activities; comply with policies; maintain courteous interactions; participate in quality improvement activities; complete mandatory education; perform other duties as needed.
