Requirements
  • A Bachelor of Arts degree is preferred, or an Associate of Science degree in Human Services with two additional years of experience in community-based case management services, or equivalent experience.
  • Two years of experience administering case management services.
  • Other combinations of education and experience demonstrating the ability to perform the services may meet the requirements.
  • Bilingual Spanish ability is a plus.
Responsibilities
  • Provide intensive case management services to eligible participants for energy assistance programs, including intake, screening, assessment, referrals, monitoring, and recordkeeping.
  • Implement a collaborative process that engages clients in defined activities and identifies resources to support their energy needs.
  • Provide intake services at the point of entry and determine eligibility for energy services, including Operation Fuel and Elderly Energy Assistance.
  • Assess eligible clients' energy needs directly.
  • Refer clients to agencies providing supportive services based on client needs and available services.
  • Use network partnerships to help clients access services and remove barriers to program participation.
  • Train volunteer personnel in satellite intake locations.
  • Provide assistance and technical support to seasonal intake certification staff.
  • Assist clients with completing documentation and verify the accuracy of information.
  • Complete required forms and enter data into the recordkeeping system.
  • Submit completed forms to service providers and regulating agencies while ensuring compliance with funding guidelines.
  • Enroll clients in utility arrearage forgiveness programs.
  • Maintain the confidential client database and files in compliance with internal and external reporting requirements.
  • Provide reports to funders as appropriate.
  • Develop and maintain knowledge of community resources and needs, including statistical data and projections.
  • Develop and maintain effective relationships with state and federal agencies, network partnerships, and community groups.
  • Provide phone coverage and respond to walk-in visitors when required.
