Licensed Social Worker

We are looking for a compassionate and dedicated Licensed Social Worker who will be responsible for providing support and assistance to individuals and families in need. Responsibilities:

• Conduct assessments to determine the needs of clients

• Develop and implement individualized treatment plans

• Provide counseling and support services to individuals and families

• Connect clients with community resources and services

• Maintain accurate and up-to-date client records

• Collaborate with other healthcare professionals to provide comprehensive care

• Attend meetings and trainings as required

• Participate in quality improvement initiatives

Requirements

 • Bachelor's or Master's degree in Social Work

•  Active state license as a Licensed Social Worker
•  Knowledge of social work principles and practices

•  Cultural competence and sensitivity to diverse populations
