Requirements
  • Must possess, as a minimum, a nursing degree from an accredited college or university, or be a graduate of an approved Licensed Practical Nurse/Licensed Vocational Nurse program.
  • Must possess a current, unencumbered, active license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse in Tennessee.

Responsibilities
  • Provide direct nursing care to residents and supervise the day-to-day nursing activities performed by nursing assistants.
  • Direct the day-to-day functions of nursing assistants in accordance with applicable rules, regulations, and guidelines.
  • Ensure nursing personnel comply with facility policies, procedures, job descriptions, and nursing service procedures.
  • Review department policies, procedure manuals, and job descriptions and recommend revisions.
  • Plan shift services, programs, and activities with assigned nursing staff and support personnel.
  • Make written and oral reports and recommendations concerning shift activities.
  • Coordinate nursing services with other resident services to maintain each resident’s total care regimen.
  • Participate in the development, maintenance, and implementation of the nursing service quality assurance program.
  • Participate in facility surveys conducted by authorized government agencies.
  • Review and update resident discharge plans and assist with nursing services planning for discharge.
  • Interpret department policies and procedures for personnel, residents, visitors, and government agencies.
  • Admit, transfer, and discharge residents as required.
  • Complete accident and incident reports and maintain required census, medical, administrative, and resident records.
  • Complete admission, transfer, and discharge documentation and perform routine charting.
  • Receive telephone orders from physicians and transcribe physician orders to resident charts, medication cards, and treatment or care plans.
  • Chart nurses’ notes accurately and document residents’ care and responses.
  • Record diet orders and report discrepancies involving physician orders, diet changes, and charting errors.
  • Prepare and administer medications as ordered by physicians.
  • Verify resident identities before administering medications or treatments and prevent medication errors.
  • Maintain adequate supplies of medications, medical supplies, and equipment and report supply needs.
  • Maintain accurate narcotic records, report discrepancies, review medication cards, and dispose of drugs and narcotics according to procedure.
  • Participate in employee performance evaluations, determine shift staffing requirements, and recommend personnel actions to the Nurse Supervisor.
  • Report staffing shortages and absentee call-ins to the Nurse Supervisor.
  • Develop work assignments and provide leadership to nursing personnel assigned to the unit or shift.
  • Make daily rounds to ensure nursing personnel perform assignments according to nursing standards.
  • Ensure nursing assistants meet training requirements and nurse aide trainees work under direct licensed-nurse supervision.
  • Meet regularly with nursing personnel to identify and correct problem areas and improve services.
  • Review personnel complaints and grievances and make required reports.
  • Receive and give nursing reports at the beginning and end of shifts.
  • Report occupational exposures, suspected fraud, information-security incidents, and protected-health-information violations.
  • Inform nursing personnel about admissions and prepare rooms for new residents.
  • Greet and orient newly admitted residents and family members.
  • Make physician rounds and arrange diagnostic and therapeutic services as ordered.
  • Consult with physicians regarding resident care, treatment, and rehabilitation.
  • Review resident charts for treatments, medications, diets, and other care requirements.
  • Implement and maintain nursing objectives and standards.
  • Check that prescribed treatments are administered properly and evaluate residents’ physical and emotional status.
  • Coordinate social and activity programs with nursing schedules.
  • Notify physicians and next of kin about accidents or changes in residents’ conditions.
  • Carry out restorative and rehabilitative programs, including self-help and care.
  • Administer catheterization, tube feedings, suction, dressings, colostomy and drainage-bag care, specimen collection, vital-sign measurement, range-of-motion exercises, and other professional nursing services as required.
  • Use restraints according to established policies and procedures.
  • Monitor seriously ill residents and ensure residents unable to call for help are checked frequently.
  • Check food brought by families or visitors against residents’ dietary allowances.
  • Ensure direct care follows residents’ care plans and wishes.
  • Meet with residents and family members and report problem areas to the Nurse Supervisor.
  • Arrange transportation for discharged residents and escort residents to the pickup area.
  • Inform family members of a resident’s death, contact funeral homes when requested, and follow post-mortem procedures.
  • Develop, plan, conduct, and schedule in-service training classes.
  • Implement and maintain orientation programs for new employees.
  • Assist with standardizing work methods and training personnel regarding blood and body-fluid exposure tasks.
  • Attend required external, facility, continuing-education, and advance-directive training.
  • Monitor personnel compliance with safety regulations, dress codes, infection-control procedures, hand hygiene, and use of personal protective equipment.
  • Maintain clean and sanitary nursing work areas, resident-care rooms, and treatment areas.
  • Participate in infection-control programs and emergency, fire-safety, and disaster-preparedness drills.
  • Ensure adequate supplies of personal protective equipment, medications, medical supplies, and equipment.
  • Ensure only trained and authorized personnel operate nursing equipment safely and efficiently.
  • Maintain material safety data sheets for hazardous chemicals.
  • Review care plans daily, identify needed changes, and ensure nurses’ notes reflect care-plan implementation.
  • Review resident care plans and ensure certified nursing assistants follow them.
  • Assist with Minimum Data Set assessments, Resident Assessment Protocols, and care-plan coordination.
  • Provide requested data to the Quality Assurance and Assessment Committee.

PACS Group has announced that co-founder and chief financial officer Mark Hancock will retire on 30 June 2026. He will continue serving on the company's board of directors as vice chairman.
