Healthmap Solutions helps health plans, ACOs, and provider groups manage kidney health across populations. It identifies patients with chronic kidney disease (CKD) and end-stage renal disease (ESRD) earlier in the disease, then uses data analytics, machine learning, and clinical know-how to guide care. The company offers predictive analytics to flag at-risk patients, care management programs to support patients and their doctors, and access to a network of nephrologists and other specialists. It charges clients per member per month (PMPM) or via shared savings, aligning incentives with cost reduction. Compared with others, Healthmap specializes in kidney disease and combines analytics, clinical support, and a nephrology network within a value-based care framework. The overall goal is to slow CKD/ESRD progression, improve patient care, and lower costs for its clients.
