Requirements
  • A current Registered Nurse license in New York State is required.
  • A minimum of three years of clinical experience in the assigned service is required.
  • Effective communication, facilitation, and organizational skills are required.
  • The ability to solve problems creatively and assertively, apply critical thinking, perform systems planning, and manage patient care is required.
  • The ability to work independently and adapt to a changing environment is required.
  • Basic knowledge of computer systems and skills applicable to the utilization review process are required.
Responsibilities
  • Facilitate the interdisciplinary team to plan, coordinate, implement, and evaluate patient care for the assigned service line across the continuum of care.
  • Work proactively with quality improvement teams, patient care standards, Social Work, and utilization management to coordinate appropriate resource use and achieve clinical and financial outcomes.
  • Participate in maintaining quality care and performance improvement through leadership, problem solving, decision making, and outcome measurement.
  • Function as a clinician, consultant, advocate, and educator for the assigned service and as a resource for the health care team, community, patients, families, and payers.
  • Assist the admitting physician and interdisciplinary teams in coordinating care across the continuum in the hospital before and after surgery.
  • Monitor patients' clinical progress using patient care standards and evidence-based guidelines to ensure timely and appropriate interventions within length-of-stay and financial constraints.
  • Assess discharge planning needs with the primary nurse, coordinate appropriate resources, and evaluate the effectiveness of the discharge plan beginning at admission.
  • Collaborate with the health care team and appropriate departments to manage pre-admission, discharge, post-discharge planning, length of stay, and utilization of resources.
  • Use specialized knowledge and evidence-based guidelines to provide leadership and guidance in developing individualized multidisciplinary plans of care covering pre-hospitalization, acute hospital care, discharge education, transition to home, and community resources.
  • Facilitate and participate in care conferences for patients with complex problems.
  • Facilitate patient and family education and the discharge process to promote continuity of care and optimal patient outcomes.
  • Apply the referral process and use community resources.
  • Review admission-screening data to clarify diagnoses, establish appropriate length of stay, identify potential outliers, and determine admission appropriateness using institutional standards and evidence-based guidelines.
  • Contact payers to confirm or negotiate benefits and provide concurrent reviews.
  • Identify capitated patients to determine appropriate utilization of services and coordinate post-hospital care using defined standards.
  • Identify high-risk patients using clinical and financial criteria and collaborate with patient financial services to problem-solve available resources.
  • Ensure that appropriate medical and legal documentation is contained in patient records.
  • Comply with third-party payer regulations, including notices of non-coverage, reinstatement, and continued stay.
  • Follow department utilization management guidelines.
  • Implement strategies with the health care team to reduce length of stay and resource consumption while optimizing patient health status.
  • Assess educational needs and provide learning opportunities for health care professionals regarding particular cases and patient care groups.
  • Develop programs or projects using creativity and critical thinking.
  • Work with the quality improvement team to continuously improve patient care outcomes while increasing quality and decreasing cost.
  • Ensure communication and transmission of documentation to community agencies.
  • Compile and report aggregate variances and data for specific patient care services with case management leadership.
  • Analyze aggregate variances with the health care team and develop strategies for variance reduction.
Desired Qualifications
  • A bachelor's degree is preferred but not required.
  • Acute care nursing and case management experience is preferred.
  • Recent experience in case management, utilization management, and/or discharge planning or home care in a high-volume acute care hospital is preferred.
  • PRI and Case Management certification are preferred.
