Requirements
  • A diploma from an accredited nursing program is required.
  • A valid and active Registered Nursing license issued by the Oregon Board of Nursing is required.
  • The candidate must be able to articulate cases through effective summaries.
  • The candidate must be able and willing to travel, usually by car, within the assigned geographic area.
  • A valid driver’s license and car insurance are required.
  • The candidate must be proficient in common software and web-based applications, including case management systems and Microsoft Office, and be able to teach others.
  • The candidate must provide a high level of customer service through empathy, active listening, and patience.
Responsibilities
  • Conduct initial in-person clinical assessments and coordinate service activation while following required forms and standard procedures.
  • Develop, implement, and manage patients’ activity plans.
  • Revise activity plans and document changes as needed.
  • Complete client documentation, including written records, reports, and notes, accurately, timely, completely, concisely, and legibly in accordance with agency policy and procedure.
  • Coordinate client care through communication and written documentation with physicians, other disciplines involved in the client’s care, and community resources.
  • Provide ongoing coaching to drive effective use of established best practices, encourage PCA education, and connect families with appropriate resources.
  • Review caregiver notes daily and appropriately address changes in the consumer’s condition or environment.
  • Ensure continuous compliance with regulatory standards and company policies and procedures.
  • Perform other job-related duties as required or directed by the supervisor.
